After dumping a haul truck load of muck, the operator thought the box was completely down, however it wasn't and it struck the main 6" compressed air line. The airline blew something into the cab which struck him on the left bicep causing a small laceration.

The LHD became stuck and muck spilled into the operators compartment, some struck his left hand, causing an avulsion of the index finger and bruising to his left leg. Amendment: x-ray revealed fracture in leg.
Employee was operating a truck when the truck jumped from 1st to 2nd gear. After applying the brakes, they locked up throwing ee in to the canopy-striking his head. He landed on the metal edge of the seat, bruising his tailbone
Pulled scaler into heading; started scaling. A large slab came down scaler, bounced as weight came off boom, throwing operator upward. His hard hat came off and wedged between his head and cab causing a 2 inch cut on top of his head.
