The injured employee went to talk to the scoop operator and put left hand on the side of the scoop bucket. EE didn't realize the scoop was running & the operator was pulling the ram back. It caught EE's left middle finger between the side of the bucket and the side of the ram blade which pinched EE's finger in it.

Showing all 21Employee was hauling a load of trash out with a tractor. Employee hit a bump causing their head to strike the canopy of the tractor. This accident happened on 3/9/16. Employee just missed their first day of work due to the incident on 7/18/2016.
Employee was a passenger in a moving mantrip. As the vehicle passed through an underpass, crossing under belt, a low hanging underpass guard struck the injured employee in the shoulder. Lost time accident as on 11/9/15.
Employee was operating a skid steer cleaning debris from the railroad loadout tracks. Bucket contacted a rail joint that had an elevated rail, causing the skid steer to come to an abrupt halt. This in turn caused the employee's hard hat to strike the windshield. The hard hat cut his forehead, and he required 9 sutures to close the wound.
Injured employee was operating a tractor when his tractor got stuck. He called another operator to come and pull him. As the other tractor backed up the injured employee got out of his tractor and walked around to the front and knelt down to unhook the clevis on his tractor. While he was knelt down he was struck by the trailer on the 2nd tractor.
While clearing himself from pathway of buggy cable, operator turned on buggy and the cable caught employees' leg and flipped him.
Employee was hauling a load of concrete blocks down the slope A with a fletcher tractor and trailer. As he turned at the bottom of slope A- he slid and hit the corner at the bottom of the slope A. He was taken to the hospital, he broke his femur bone on his left leg, they operated on his leg and put a pin in it. He is being released from the hospital today 5-10-13.
Employee was running a bobcat and there was some cables hanging down in front of him and he tried to raise them up with his left hand and mashed his left hand between the bobcat and roof above him. We took him to the hospital and he had 2 pins put in his left hand.
Employee was running the bobcat and when he was getting back in the seat he hit his head on the front of the equipment jamming his neck.
Employee was shoveling the slope belt, smacked his shovel against a top roller to clean the mud from it. The belt caught the shovel causing it to flip over and strike him on the right forearm.
The injured contractor was standing by the air lock doors. The trailer on the truck coming through the doors caught the door & the tire of the trailer ran over the contractors ankle.
Employee was working on the beltline in the tailgate section of M-Class mine when he was attempting to ascend belt structure. Employee suffered a dislocation of his left knee.

1" air line caught on bucket lifting pump into air. The pump slid off and fell 15' hitting employee on collar bone and right ankle. Collar bone is broken and ankle is bruised.
EE was trying to dump a load of rock. Rock was wet & frozen inside trailer. Hydraulic cylinder broke & trailer fell on cab of truck.
