Maintenance tech was tracing leaks on hydraulic cooler of Haul truck. Unit was running, tech was looking to pinpoint leak, when working around fan area, the cooler leak opened up. Tech instinctively stuck hand out to block from being sprayed. The rag in tech's hand was sucked into fan & pulled hand into fan. The tech received a laceration to fingers that required stitches.
Two utility workers were hanging high-voltage electrical cable in the O3-N roadway. They were using the manbasket of a telehandler to lift the cable while hangers were placed when the employee asked the telehandler operator to raise the basket more. As the basket was raised, the employee's hand was pinched between the basket rail and the roof of the mine.
Employee was driving haul truck in the 261 area when they accidentally engaged the E-stop on the truck causing the truck to come to an abrupt stop. As the truck stopped suddenly, the cinching style lap-belt came un-threaded from the male end of the buckle allowing the employee to strike the windshield of the truck.
The employee's hand slipped off the door of the AC condensor, tried to catch the door with the left hand and electric fan blade make contact with the hand, causing a laceration.
EE caught between fuel truck's open door & telehandler when telehandler backed up towards fuel truck. EE pinned at their rib area and was released as the telehandler operator pulled forward. EE taken to local ER, first aid only at the time of the accident. This became reportable to MSHA on 6/20/19 when they received med treatment of root canal and crown to a permanent tooth.
At approximately 5:30pm at the Immel Mine 484 track level bulkhead, the chute pullers had just dumped their rail cars and were heading back to the chute to fill their cars again. As they traveled through the bulkhead area the passenger's head came in contact with the bulkhead structure.
