Showing all 3Employee was operating bench loader 2754 removing material close to a 30' face in phase 2. A rock, about 4' to 5' in diameter, slid down a 15' section and struck the edge of the loader bucket causing the equipment and operator to jerk. About an hour later the base plate broke off the seat and employee reported that his back was sore and left for the night.
While operating a skid steer loader cleaning material by conveyor 34 counterweight, the bucket of the loader hit a concrete slab that was hidden under material build-up and caused the loader and operator to jerk forward. EE's hardhat flew off, bounced off the loader and hit the operator in the forehead. Possible strain to neck and upper back.
