Struck by falling object

Miner was unplugging a chute in the secondary finishing plant when a rock fell and hit the top of EE's right hand, causing swelling and bruising.

Miner was using an impact drill and hand wrench to loosen bolts on a side guard rail when EE's finger got trapped between the steel frame and wrench.

Struck against stationary object

Miner was throwing a chain over a beam while standing on a step ladder. Chain slipped, yanked their hand. Miner's left hand hit the ladder forcefully resulting in a laceration of the top of left thumb.

Miner was doing a pre work inspection on wheel loader. Miner climbed a ladder to a platform on the rear of the loader to check fluids. Miner then climbed from the platform on to the tire of the loader, instead of going back down the ladder and attempted to get into the operator's cab and twisted a knee.

Struck against a moving object

Miner was operating a wheel loader to load the primary plant crusher when they ran over a large rock. This caused the loader to slam down with tremendous force, jarring the lower back of the miner and causing pain.

Miner was trying to lift a full tank of acetylene in to the bed of service truck when they strained lower back.

Fall from machine

Miner came in to the shop for a lunch break and was exiting the water wagon with EE's lunch cooler in EE's hand. EE's left foot slipped off the top step of the water wagon and fell to the ground, landing on EE's left shoulder.

Miner stepped on an unsecured 2 X 4 wood brace support for a foundation precast, when it gave way causing miner to fall against the compacted sub-base embankment, striking the right side of ribs. Miner sustained right side 9th rib fracture and is on modified duty.

Over-exertion (Not Elsewhere Classified)

Miner was welding plates in the reject chutes of Tower 23 and welding overhead with head tilted in a left position within a two and a half (2.5) hour period. On 9/06/2018 ee reported pain to this left side of neck.

Fall to the walkway or working surface

After adjusting the Hazemag, Miner stepped onto the motor and slipped onto the working platform. EE landed on EE's right leg bearing most of EE's weight and felt immediate pain to EE's right knee.

Struck by flying object

Miner was using a 2 ton chain come-a-long to jack up a shaker and change a motor mount spring, when the come-a-long chain broke. The chain struck left elbow and the block (housing) struck right forearm and right leg.

While Lifting a 35 gallon grease barrel from the ground onto the flatbed truck (#290), employee strained right shoulder. Barrel weighed approximately 80lbs.

While working at the Hazemag crusher, just finished turning the blow bar and started to remove brackets. Co-worker, proceeded to push the bracket back and ee left pinky finger was struck by the bracket

Struck against a moving object

While scrapping the ground, the loader made a sudden stop when the bucket hit a rock. The impact caused the head light to fall off the bracket and the operators body to jerk. Immediately thereafter, employee felt pain to his neck

Over-exertion in lifting objects

Mounting steel plate to conveyor belt on the shop floor. EE lifted a 4'x 8' steel plate off the floor to stand it up and slide pallet jack under it, EE felt some strain. Later that day, while mounting 2nd steel plate & moving plate by hand with pry bar, EE felt a sharp pain in lower back.

Fall from machine

Miner was stepping down onto the last rung of the ladder on the CAT 988B Loader (equip#2746), when the weld of the rung broke. Miner fell approximately 2 feet to the ground on his back, followed by his head. Miner felt ok to work and 2 hours later he felt dizzy. Miner taken to the medical clinic to get checked.

Miner stepped off Tower 23 concrete slab onto uneven ground, when he fell and twisted his left knee and over extended his right foot. Miner fell on same level.

Struck against a moving object

While operating a skid steer loader cleaning material by conveyor 34 counterweight, the bucket of the loader hit a concrete slab that was hidden under material build-up and caused the loader and operator to jerk forward. EE's hardhat flew off, bounced off the loader and hit the operator in the forehead. Possible strain to neck and upper back.

While trying to lift a tire from the service truck (#1291) that was lying flat on the truck bed, employee felt a sharp pain to his lower right side of his back

Fall from scaffolds, walkways, platforms

Our employee broke his upper shin bone below his knee on his right leg while repairing a 988B Cat Loader for Ameron. The 4 ft. platform he used to gain access to an area to replace a pin collapsed. He is waiting to have surgery.

IE & another employee were working in a large discharge chute removing rocks to clear a rock jam. Employees were casting 4"" to 11"" basaltic rocks out of their respective sides of the box. The other employee attempted to cast a larger size rock over the side of the box that IE was positioned. The rock did not clear over the side, fell & struck him on his left shoulder.

Struck against stationary object

While clearing an area by Tower 23, employee was piling debris off to the side to make room for the crane when he threw a metal box tubing, which was part of an old walkway cage and the jagged edge cut into his right palm, leaving a deep laceration.

Fall down stairs

After punching out for the day, employee was descending down from the stairway when he lost his footing from the middle landing of the steps and tumbled down the remaining 9 steps.

Struck by falling object

While installing new plates in the F2 hopper with another co-worker, employee began to install the 2nd angle iron when he noticed welding debris in the area to place angle iron. EE leaned the angle iron slanted against the 1st angle iron to clean debris when the 2nd angle iron slipped and smashed his right hand middle and pointer fingers between the ledge and iron plate.

While installing pads in the F1 feeder, EE was holding one end of the pad while other employees were inserting bolts to attach pad to the links. In the process, EE's left hand got wedged between two pads causing the pad to cut and smash the tip of his middle finger.

While refueling the D8 Dozer at the fuel station, employee was descending from the equipment with his right hand on the dozer's railing. As he reached out to grab the fuel nozzle with his left hand, his left foot slipped causing a jerking motion unexpectedly to his right arm. Thereafter he started to feel mild pain. Incident reported on 4/02/08

While walking up the stairs on T-23, employee was pulling a 1/4"" X 95' grease hose when it got caught on an old screen lying on the ground, causing his right wrist and shoulder to jerk in a backwards motion.

EE was attempting to move an extended jack stand and in the process, he released the jack stand, the force of which caught the EE's baby finger on the way down. Injury was a laceration/contusion to right baby finger, originally a non recordable due to no lost time with a one time first aid treatment, however it should have been a recordable injury, restrictions on hand use given.

While repairing the master screener, employee was loosening the fitting valve when it popped loose causing his right arm to jerk back and hit the frame of the machine.

After pumping fuel, employee slipped off platform near cab door, falling approximately 2' and landed on the dozer tracks hitting his right arm on the grab handle and exterior wall of the cab.

While operating loader #2745 on uneven ground, back and right side ribcage area began to hurt while trying to hold self in position on the seat. The loader would stop suddenly at full turn of the steering wheel.

While standing on the rock shelf in C-13 head box cleaning build up with a long hooked rod, EE felt a pain in lower back.

EE was coming down from the top level of Tower 23 and walking on the catwalk of the second deck, when the lugs on the soles of ee's shoe got snagged in the opening on the expanded metal grating. EE stopped in his tracks. He pulled his back foot free and in the same motion kicked his shin on the lower railing.

WHEN REMOVING WORN TOP PLATES OFF AS-1 TABLE, EE TRIED TO STOP A SECTION OF THE PLATE FROM FALLING; CUTTING HIS RT 5TH FINGER FROM PALM TO FINGERTIP.

WHILE EE WAS CLEANING SECONDARY PLANT C13 CONVEYOR, BELT WAS RUNNING OFF TO ONE SIDE; COUNTERWEIGHT ROLLER HAD MUD BUILDUP. EE TRIED TO CLEAN ROLLER PULLEY WITH A SMALL HOE; WHILE HOLDING ONTO HOE HIS LEFT HAND GOT CAUGHT IN THE PINCH-ROLLERCAUSING TRAUMATIC AVULSION TO HIS LEFT 3RD FINGERNAIL, MULTIPLE CONTUSION/BRUISES AND SMALL CUTSON LEFT FOREARM.

EE WAS CLEANING THE RETURN ROLLERS FROM C20 CONVEYOR AND HE SLIPPED. WHILE HE WAS FALLING, HE GRABBED THE SAFETY GUARD AND THE WIRE FROM THE SAFETY GUARD PUNCTURED THRU HIS GLOVE AND BETWEEN H IS TWO FINGERS. LACERATION ON RIGHT HAND LONG FINGER SUTURED IN EMERGENCY ROOM.
