Showing all 4EE was operating an excavator when the haul truck EE just loaded rolled into the cabin of the excavator EE was operating. When the excavator was struck, this caused the windshield to shatter and debris flew into EE's eyes. EE was taken to the emergency room where EE's eyes were flushed and EE was sent home.
EE was operating the skid steer when a rock came over the bucket and shattered the windshield. The rock landed in EE's lap and a small piece of glass got behind EE's safety glasses and into left eye. EE used bottled water to flush out the eye and was sent to urgent care where they flushed the eye and confirmed that it was scratched, EE was prescribed an antibiotic ointment.
EE did not follow safety procedures of lockout and tagout, and proceeded to adjust door on hopper while conveyor belts were in motion.
