Employee reported having back pain to EE's supervisor. EE was about to grease EE's loader, but hadn't started yet, when EE began to feel pain. EE does not know what caused it. EE was not doing anything when the pain started. Axis care line was called. Nurse advised rest, OTC pain meds, & Ice therapy. Employee was sent home at 12:40 to rest and follow nurses care plan.

Showing all 21The employee was hauling material down a steep slope at the quarry when they lost use of the brakes and the vehicle turned over, resulting in a fractured finger.
Employee was disconnecting a 4" water line from the water truck when the water line flew back and made contact with the employee's mouth, cutting EE's lip and chipping a tooth.
Employee was pulling electrical wire in an awkward position when EE pulled a muscle in EE's left shoulder.
Employee was climbing up steps of a rented loader, missed the top step, and fell to the ground injuring left hip, left knee, and left elbow.
The employee sat in the passenger seat of the supervisor's truck while a CB was being installed. It was at this time the employee experienced a possible personal medical event and fell out of the truck, striking their head. This caused a small laceration/abrasion to the right forehead which involved swelling.
EE texted Plant supervisor that the excavator operator was dropping large rocks too hard in EE's haul truck. EE did not report and injury at that time but after EE's shift EE called HR and said EE's back hurt. EE to Texas med clinic and the ER and was diagnosed with possible muscle and vertebrae injuries.
IE on the dredge and EE on the bank preparing to fuel the dredge. While lowering the fuel hose to the dredge, IE attempted to catch the hose and the shut off valve cut right palm. EE transported IE to Urgent Care where IE received a Tetanus shot and 5 stitches and was prescribed antibiotics. IE returned to work and finished out the day.
EE was changing out screens in the shaker at approximately 8am when EE hurt EE's left wrist. EE alleges that EE heard and felt a "pop" while holding up the screen ( 150 lbs ).
EE was operating an excavator when the haul truck EE just loaded rolled into the cabin of the excavator EE was operating. When the excavator was struck, this caused the windshield to shatter and debris flew into EE's eyes. EE was taken to the emergency room where EE's eyes were flushed and EE was sent home.
Employee was filling the water truck when the flexible hose got stuck on the fill pipe. Employee pulled the pipe and it released suddenly striking them in the upper lip causing a small laceration and chipped tooth.
EE was operating the skid steer when a rock came over the bucket and shattered the windshield. The rock landed in EE's lap and a small piece of glass got behind EE's safety glasses and into left eye. EE used bottled water to flush out the eye and was sent to urgent care where they flushed the eye and confirmed that it was scratched, EE was prescribed an antibiotic ointment.
Miner was climbing into a skid steer. Miner's boots were muddy from the rain. Miner slipped & struck right knee on the bucket of the skid steer. The initial pain did not subside, miner was sent to be evaluated. The miner was seen at a local urgent care. X-ray showed no broken bones. Miner has tenderness & restricted motion. Restrictions given for walking, bending & lifting.
On Friday, November 19, 2021 sometime between 8-10am EE was welding and grinding at the quarry plant. EE was grinding a spot for my ground for the welder. EE welded 2 bolts to put up a piece of rubber. EE felt like there was something in EE's left eye, but was not able to see anything. Saturday morning, EE woke up with a blood shot and swollen left eye.
While servicing the dredge, EE slipped and hit leg on a 10" pipe causing an injury below the knee. It was swelling so EE went to get it checked to make sure there was not any damage. Returned right back to work.
Using excess force to loosen a bolt with a wrench
EE did not follow safety procedures of lockout and tagout, and proceeded to adjust door on hopper while conveyor belts were in motion.
Working too closely to the edge of a boom truck, employee slipped off the side. Sore ribs
One employee was operating the excavator. Second employee's truck was stuck and employee one used the excavator to push truck out. Employee two alleges that it popped their neck.
While working on the conveyor, IE was taking out a Carter Key, there was a wasp, another EE said "Watch out!" IE jumped, tried to run, tripped and fell on IE's arm/shoulder.
Employee was getting out of truck to get weight ticket and slipped on gravel, employee grabbed a piece of pipe that was sticking out of the ground in an attempt to arrest fall. EE suffered a cut to the palm of left hand resulting in receiving 9 stitches.
