Employee was using a crane outfitted with a magnet lifting device attempting to lift a cutting edge from a dozer. When the magnet was attached, the employee stuck EE's face near the magnet and the handle released and hit EE in the nose causing injury.

The operator was driving a surface loader and hit a bump, which jostled them. They didn't immediately feel pain and finished the shift, but at the start of shift the following day they reported soreness in their back to their supervisor. They were sent to the hospital, where the doctor wrote them out of work for two days to rest their back muscles.

An UG haul truck operator injured EE's back while dumping CRF material into an underground CRF bay. While the dump bed was raised up, it scraped the "back"/roof of the CRF bay, causing the cab of the truck to lift off the ground and then fall back to the ground.

On May 5, employee was being trained to drive a Cat 777 haul truck. While dumping, the truck had a jarring motion. The EE complained of neck and back pain to their trainer. EE was examined by a doctor and was RTW on restricted duty. The EE reported to their next shift on May 10. EE failed to successfully complete the training program and left employment on May 11.

CAT 745C operator was driving to park at shift end. Drove off designated haul road to cut across fill area and hit 24" deep ditch. EE bounced up and hit head on the grab bar behind the seat, sustaining a 4" laceration to back of head. CAT's onboard monitoring system indicated that driver was wearing EE's seatbelt. EE was transported to the hospital where EE received 14 sutures.

An operator of a haul truck over turned on county road 188. The truck left the permitted area via the county road when it ran over a rough patch of pavement and over turned. The seat belted driver sustained a laceration to the back of his head which required sutures; in addition he spent the night in observation at the hospital. He returned to work 2 days later.
