Employee was in the process of loosening a hydraulic hose on a low trac on the 004 MMU. While loosening the hose the pallet forks leaked off squeezing EE's head against the tilt arm of the low trac and the low trac frame. This resulted in 6 stitches.
Employee was sitting in the rear seat of the mantrip. During investigation it was determined EE re-adjusted EE's position in the seat and contacted a bolt installed in the mine roof.
Employee was asleep while riding on #7 diesel mantrip, when the operator moved over to the right to allow clearance for a pick-up truck to pass. Employee's foot was caught between a jersey barrier and canopy post.
Employee stated they were applying brakes, when the front end jerked causing them to run under the moving slope belt.
EE stated that while operating the 21SC shuttle car being loaded by the continuous mining machine, the bumper of the CM came in to contact with load end of the shuttle car. When the car operator pulled away, this caused the dump in to be raised slightly then fall suddenly, jarring the car operator resulting in EE's head hitting EE's head on canopy.
While lapping belt EE hooked up a pallet puller to a 15' piece of 60" belt, when the scoop operator began to pull, the belt flipped up knocking EE to the ground, landing on left shoulder. "Employee continued working until 06/09/2025".
Employee alleged they were sitting on mantrip and raised up to adjust their belt. Through investigation, it was determined the employee was positioned on their knees and raised up to get their dinner bucket when they came into contact with the metal beam, resulting in a laceration requiring 4 stitches.
Employee stated while backing employee's 4-wheeler over to 1-A belt, employee's left thumb was caught between the steering wheel and an elevated fire valve that was pointing downward.
Employee was riding on the #20 mac 8 ride heading to the #2 section, when the operator started accelerating wide open and came in contact with the coal rib causing the mac ride to come to a abrupt stop. resulting back pain to the employee.
The employee, an equipment operator, felt like something was in eye and the eye became irritated. The employee flushed the eye and continued to work till end of shift. On 4/1/2025 the employee returned to work but continued to feel discomfort. The employee went to medical facility and it was found that the eye had a scratch. Meds were prescribed.
Employee was a passenger (driver's side rear) on a 4-man mantrip traveling outside at the end of shift. The mantrip hit a hole in the road, measuring 36" wide x 36" long and 2-3" in depth, in the travel way and the employee shouted to stop. When the mantrip stopped the employee fell out of the ride. The employee complained of lower back pain, and numbness in left leg.
The injured employee was in a CAT785 Rock Truck that was being loaded by a CAT993K Loader. The loader ran over a rock causing the loader bucket strike the truck bed. This caused the injured employee to be rocked back and forth, causing neck pain.
Torque Contractor was coming off dump hill and hit jake brake. Acted like it went from 3rd stage to 1st stage and jerked EE's neck. First day missed was 12/10/2024.
Employee was trying to remove a fly pad out of the scoops ram which was all the way out, the scoop operator didn't see employee and began to pull the ram in catching EE's fingers in between the ram and scoop bucket.
Employee was adjusting the width on the fork spacing to pick up a pallet of block with fork lift on lo-trac. When sliding fork in, EE caught EE's left ring finger between the fork and the frame of the fork lift on the lo-trac. This resulted in 3 stitches.
Employee stated EE smashed finger between 4" I beam and scoop bucket. EE finished EE's shift and sought medical treatment. X Ray revealed broke R index digit. EE did not miss any scheduled shifts.
Employee was using machine mounted re-trackers on a Brookeville locomotive due to the machine being off the track. The employee was operating the re-trackers from outside of the operator's deck of the machine. Once the locomotive was back on the track it rolled due to the brakes not being set catching the employee in a pinch point between the machine and the mine rib.
Employee was in process of installing a keeper on the left side tie rod end on a battery four wheeler, as EE reached up to get EE's channelocks , EE made contact with the foot pedal causing the machine to reverse striking EE's left leg & abdomen, EE continued to work until 9-16-24
Employee was watching 8" Poly Pipe as Motor was pulling it up the track entry when the tail of the pipe swung around striking the employee in the left leg.
Employee was unhooking chain from buggy that was being pulled with a scoop. As they were unhooking the chain the scoop inadvertently rolled and as they pulled their hand out the cotter pin caused a laceration to their right index finger and thumb.
Contractor was riding in mantrip. EE got up on knees to adjust SCSR and to put coat on and grabbed side of mantrip. Mantrip was pulling out and smashed EE's hand between the mantrip and rib on the corner.
EE stated EE was riding in mantrip when the operator lost brakes and derailed. The mantrip came off track and EE hit EE's head and stated EE's back was hurting.
Employee was descending the Pax haul road at the #7 marker, EE stated EE lost EE's brakes. EE then guided the truck which was loaded into a sump causing the truck to stop. EE struck face, head on the steering wheel causing a broken nose and right eye orbital fracture. EE was wearing seatbelt and road conditions did not play a part in the accident.
EE was watching cathead as it was being moved with scoop, it came unhooked and EE told Co-Worker to back up scoop, when backing scoop up Co-Worker hit EE in both legs resulting in a torn muscle in the left leg below knee.
Employee stated that when they were tramming a rubber tired ride through a curtain that it knocked their safety glasses off and the curtain came in contact with their eye.
Employee stuck a finger between a moving roller and a sensor mounted on structure. This resulted in a fracture to the left index finger and a laceration requiring 5 stitches.
Tramming scoop in track entry had rt arm arm outside operator's compartment, ran scoop into rib catching arm between scoop and rib causing laceration, resulting in sutures needed to repair the wounds
Employee pulled 12-man mantrip up to intersection to hook up cable and didn't set park brake or turn the ride off. Employee got in front of ride with back toward the ride. The ride began to roll and contacted lower part of left leg resulting in a fracture.
While being transported to section on rail mounted vehicle, employee's knee was protruding outside the confines of the passenger compartment and came in contact with a timber set alongside the belt line.
Employee was working near the section conveyor tailpiece, when a rock came through the tail piece knocking the guard loose striking the employee causing a laceration around left eye. Resulted in 16 sutures.
Employee had hand on canopy of underground forklift while operating. Canopy contacted mine rib pinching left hand. Resulted in a laceration requiring 13 sutures.
Rib rolled out beside of track. Miner was riding personnel carrier with feet on edge of frame. Rib roll caught left foot between rock and frame resulting in a fracture to several metatarsal bones in left foot.
Employee was helping remove a set of batteries out of battery tray on surface using a lifting beam hooked to the forklift, two other jacks was being used on each side to aid in raising battery out of tray, as they bent over to move the jack the lifting beam came unhooked hitting employee in the face, employee was taken to hospital and diagnosed with sinus cavity fracture
Employee was welding a piece of metal on a sled when the tractor trailer started backing up and ran over employees right leg
Injured employee was walking on the opposite side of the newly installed refuse belt from a telehandler that was picking up new belt lying on the ground to place on the belt rollers. The belt slid off the forks and side of belt structure bumping the employee enough to cause employee to lose balance in the mud, twisting a knee.
Employee stated employee was cleaning the face and was backing up scoop to set over, employee placed left hand on top of the operator door and got the deck into the rib smashing hand between rib and scoop door causing fractured thumb.
EE was walking to drop cable slack in preparing for power move, an electrician was trouble shooting reel on the #2 car, when the electrician pecked on the valve the cable reel engaged as EE was stepping across cable, it came up striking EE on the mouth area causing laceration to lip requiring couple stitches.
Employee was utilizing a 1/2 inch chain connected to a service scoop to pull slack out of a feeder conveyor chain. The 1/2 inch chain broke striking employee in the face. Resulted in a fracture, stitches, and a chipped tooth.
While operating a scoop, injured ran over a rib roll, jarring back and neck in the operator's compartment of the scoop.
Employee was hanging miner cable and waterline, shuttle car was leaving the continuous miner when it struck the miner cable and water line, knocking employee to the ground, injuring wrist.
EE was operating the # 19 4-Wheeler and when it powered out and lost power on the hill leaving the portal. It then started to roll backward while EE was trying to reset the tram, but EE then steered the 4-wheeler into the bank. The 4-wheeler climbed the bank, rolled over, and pinned EE underneath it.
While cleaning No. 9 entry on No. 1 section, employee peeled up a large floor rock measuring approximately 38" wide x 40" long x 6" thick. The rock flipped up and partially entered the operators deck striking employee on the side of the head. Resulted in a laceration of EE's ear requiring surgery.
While tramming the scoop, the door to the operator's deck swung open. Employee attempted to shut door back when the scoop brakes suddenly locked up due to the door being open causing the door to swing shut pinching left middle finger between door and frame. Resulted in amputation of finger to first joint. 75 days standard charge.
Employee came in contact with scoop causing a laceration to the back of EE's lower right leg.
Employee stated was moving timbers when a shuttle car ran over a timber butt causing it to fly up and hit EE in the chin.
While loading a gear case for a continuous miner onto a flat car underground, the gear case slid off of the scoop bucket causing the scoop to bounce. Employee hit EE's head on the canopy inside the operators deck.
While operating a shuttle car, employee pinched fingers on right hand between the canopy of the machine and the rib, resulting in a laceration that required stitches.
Employee was guiding scoop operator in to scoop up a bundle of conveyor belt when the scoop bucket struck the employee's right foot.
Night shift driver of 036 tractor at Davey Branch Mine was starting the engine on the bed treatment rack. EE's right hand came back and struck the container cage, peeling EE's skin off 3/4 to 1 inch across the top of the hand.
Employee was operating a rail mounted personnel carrier underground when it collided with an 15 ton locomotive near No. 5 Head.
