HWM forklift operator had the water tank on the forks. In the process of going to the water tree to fill it up, EE struck a rock in the road causing EE's seat to bottom out giving EE a sharp pain in their neck and back.

EE was cleaning the pit floor when a rock fell from the highwall and came through the windshield of the Komatsu WA600 loader EE was operating. Rock struck EE's right leg causing bruises/contusions, glass in EE's eyes, small lacerations.

The employee was attempting to start the lube truck by jumping across the starter when the key switch ignition did not work. The truck was in gear and it lurched forward onto the employee's left leg just below the knee. Employee sustained a mashing injury to leg. The truck did not go across employee's leg, just caught the outside of the leg.

Victim was operating assigned loader. It had been raining a substantial amount. A rock fell from the highwall and struck the windshield of the loader causing dust, dirt & glass to get in EE's eyes. EMT's flushed and bandaged EE's eyes. At the hospital no foreign material was found in EE's eyes but there was minor scratches. Ointment was applied and EE was released.

Employee was utilizing a Cat 980-G loader to stockpile coal as it came from the highwall miner. EE backed off the stockpile too fast and when EE contacted the pit floor it caused EE's neck to be jerked. EE states EE heard neck 'pop' and it began to hurt. EE finished shift, went to the doctor the next morning then reported for work.
