As the Trac Mobile, pushing one rail car, moved down the track, Operator B walked alongside, outside the danger zone, to align the rail car with the scale. After it halted, the rail car uncoupled from the Trac Mobile and continued rolling down the tracks. Operator B ran to get ahead of the rail car to apply the manual brake, but Operator B's foot got caught under the wheelset.

Two haul trucks, one loaded and the other empty, were passing from opposite directions on curve south of mining unit stockpile area. Loaded truck continued in a straight line instead of turning into curve and sideswiped unloaded truck. Driver of unloaded truck bit into the tongue on impact and required stitches to close wound.
