During regular duties the operator of the dozer stopped, exited the cab, and the dozer rolled back crushing the operator.

Fall to the walkway or working surface

On 2-2-26 While EE was exiting personal vehicle, EE slipped and fell on patch of ice and EE was evaluated at onsite clinic and returned to normal shift with no restrictions. On 2-12-26 EE reported pain in lower back, EE was referred to an offsite clinic. EE was diagnosed with a fracture in L1 vertebra. EE has been given no restrictions and has no lost time to report.

Fall to the walkway or working surface

Employee was walking in front of the admin bldg. when their left foot caught the edge of the pavement and fell on their left side. Hurting their left arm and scraping the side of their cheek and chin.

While descending the screener stairs, the operator was carrying a chain hoist in one hand and using the other to hold the handrail. As they reached the bottom of the stairs, the operator stepped onto a hose that was lying across the walkway, causing them to twist their right ankle.

Over-exertion in pulling or pushing objects

Driver was pulling on a binder with EE's right hand, securing a machine EE had just loaded and felt a pop in EE's wrist followed with some pain. Driver called dispatcher and explained incident. 3/25/2025 the driver said it was bothering EE with the same amount of pain and wanted to report it. Employee placed on restrictions 4/1; RTW 4/4

Caught in, under or between a moving and a stationary object

As the Trac Mobile, pushing one rail car, moved down the track, Operator B walked alongside, outside the danger zone, to align the rail car with the scale. After it halted, the rail car uncoupled from the Trac Mobile and continued rolling down the tracks. Operator B ran to get ahead of the rail car to apply the manual brake, but Operator B's foot got caught under the wheelset.

Struck against stationary object

While employee was assisting another employee unplugging a header, the employee stepped back to avoid water splashing on EE. As the employee stepped backwards, they struck the corner of a junction box injuring their right rear shoulder.

Fall onto or against objects

EE changed outer cab filter on CAT D6 and was climbing down. EE grabbed handrail with both hands before turning body to climb down. EE brought left leg down putting EE's foot on the step below. Went to bring right leg onto the step, left foot slipped off step swinging lower body towards machine. EE landed on edge of step with back of right leg resulting in laceration of right leg.

Employee was attempting to remove a long (10"") bent bolt from a steel process line on MMU B. During the attempt, the wrench slipped off the nut while the impact turned the bolt allowing employees hand to move into a pinch point area. The bent bolt then rotated pinching the employees left middle finger between the bolt and the pipe. Fracture to the left middle finger.

Employee was exiting the Caustic Office, slipped on the grating and fell onto the gravel. An MRI administered on 01/11/2024 revealed an acute fracture to the right hip.

While exiting the Wet Mill at the end of shift, employee tripped and fell when their boot contacted the metal threshold plate that transitions between the pier set and walkway plank. Employee's right knee contacted the grating causing a laceration requiring stitches.

Miner was in a pickup truck with 3 other miners when truck drove over a pothole causing discomfort in miner's back.

Miner was in sitting position, leaning forward, welding a pipe cradle. Miner stood up as the miner felt back stiffening. Miner swung right leg over 33"" high chain then left leg when miner felt discomfort in lower back.

During the hoisting of a boat engine, an employee was positioned at the rear of the engine to help guide the engine during the lift. The center of gravity shifted to the rear of the engine, where the employee was positioned, and the engine struck the employee on the leg.

Over-exertion in lifting objects

Operator was obtaining a sand sample into bucket from discharge point in pipe. Operator collected sand sample and lifted the bucket onto scale hook to weigh sample. When operator turned to the left to hang sample on scale. Operator stated they felt a pop in left knee. This was the third sample taken by the operator that shift.

Miner was sweeping floor when EE felt EE's lower back muscles tighten up. Miner has a pre-existing condition from several years ago. Doctor prescribed PT on 5-10-21.

Fall onto or against objects

AN EMPLOYEE WAS WORKING ON THE SHAKER SCREEN AT CHEMOURS. EE WAS ASSISTING IN THE WASH DOWN AREA. THE FIRE HOSE HAD NO VALVE ON THE END TO REGULATE. WHEN EE GRABBED THE HOSE THE OPERATOR OF THE FIRE TRUCK TURNED WATER ON FULL FORCE. THE FORCE OF THE WATER COMING THROUGH THE HOSE KNOCKED EE OFF BALANCE, MAKING EE FALL BACKWARDS HITTING THE BACK OF HEAD ON THE TOE BOARD.

Struck by falling object

EE was using a pry bar to roll the flange end of a 20"" diameter by 40"" long piece of rubber wrapped expandable pipe. As the EE was prying the pipe away from where EE was standing and lifted the pry bar from under the pipe, the pipe rolled backward causing the flange to contact the top of the EE's left foot. EE returned to restricted work.

Miner states they were shoveling rock on the day of the potential incident. At the end of the shift, the miner stated they felt some discomfort on the left side of their back.

Employee was shoveling rock from on top of sump. Employee went home and reported soreness in back the next day (11-5-18).

EE stated they lost balance on workboat causing them to try and catch themselves. When their hand was placed on the deck, it apparently was underneath the mooring rope and the workboat deck potentially causing the rope to crush EE's right hand. *EE's middle finger was removed during surgery.

Approx. 11 AM on 3/20/2014, a DuPont emp. was descending steps on the MX. dredge, lost their footing & slipped down the steps. The emp.'s left elbow hit the steps during the fall. The EE began to lose consciousness when they stood up. A nearby emp. assisted the EE to the deck. The EE was transported to site medical for treatment. Injury classification is ER-MTC.

On Monday Jan. 27th 2014 a Ring Power Mechanic was onsite to perform a scheduled PM on our Challenger equipment to access the engine compartment. At some point, the Mechanic let the hood go and the hood came down on the left foot of the Mechanic. The employee was taken to their company physician. This injury is currently classified as an ER-LWC

Absorption of radiations, caustics, toxic and noxious substances

An Operator reached into bolt bin, when they felt an insect. They jerked their arm back and forth and thought they had dislodged the insect. A couple of minutes later, they felt a bite on their lower abdomen. They did not report the incident until several days later when it was evident that there was a possible infection with the bite location.

At or around 6:50 pm on 10/23/2013, a DuPont Operator was in the process of clearing an obstruction from the turn table with an alignment bar when the employee's thumb was pinched between the alignment bar and structure. The EE was transported to the hospital for treatment.

At or around 8:30 am on 10/10/2013, a DuPont electrician was in the process of aligning a drive bus into the cutter drive motor when he reported soreness/crunching in his knees to his supervisor. The employee was transported to medical where he was evaluated and prescribed a prescription medication. Further medical testing conducted and no acute trauma was seen.

The employee came in contact with 12.86 PH water and received two quarter size burns on her left hip.

Struck by flying object

Approx. 3:00 PM 03/05/12, employee was using grinder to remove excess material from a welded bead on a steel pipe. Employee noticed something in right eye and reported to plant medical. Employee was referred to off-site physician that removed embedded piece of metal.

Over-exertion in lifting objects

Employee had lifted a 1 inch steel plate weighing approx. 55 lbs.(weight of steel plate was not know in the field) and was holding it up while waiting for installation. After handing off the plate and leaving the area, a few minutes later he felt pain in his lower back.

Employee slipped and his knee hit a bolt on super structure.

On 08/11/11, Employee was going up and down on ladder/stairs making repairs to process equipment. The following day, the left knee was swollen and had soreness & pain. On 08/30/2011, the employee reported to medical/supervision concerning the knee pain.

EE was working on 30"" dredge feed line piping. When he crossed over the pipe, his right knee made contact with the steel channel on the pontoon surface. He reported the incident to supervision and he said he felt stiffness in his right knee. Plant Physician recommended employee get an x-ray. X-ray results were negative. Employee was instructed to apply ice to the knee.

On Thursday 07/22/10 at 1:45 P.M., a mechanic pinched the ring finger of the right hand while replacing belts on a 600 HP pump/motor assembly. The mechanic was using an 18"" aluminum pipe wrench to turn the 48"" diameter sheave to roll the belts on when the mechanic's gloved hand came in contact with the guard support structure pinching the ring finger.

On 05/25/10, a mechanic replaced the MXWM domestic water pump. He stood up to reach for a pipe wrench & felt a sharp pain in his back. Evaluation by the plant Dr. was a pulled muscle. Injury was classified as a FAC. On 08/04/10, MRI revealed emp. has a herniated disc. Emp. will receive further evaluation. On 08/11/10, this case was re-classified as an OER-RWC.

On Friday 12/25/09 at ~11:00 P.M., a Dupont water operator sustained 2nd degree burns on his right hand and chest. when the #3 Seepage pump suction pipe failed and was sprayed with hot water from the pump. Operator was treated at the local hospital and released. A full investigation is ongoing. This case has been classified as an ER/MTC

Employee was in the process of taking down a 1 ton chainfall rigged to an overhead beam. He went up a stepladder and unhooked the chainfall setting it on top of ladder. As he started down the ladder the chainfall slipped off the ladder. As he tried to catch the chainfall the hook caught his glove and he suffer a jerk and strain to his left shoulder.

On Tuesday July 21, at approximately 10:15 in the morning a DuPont operator received a laceration on the right thumb when he was working on a magnet. Employee notified supervision and was examined by Plant Medical, then transported to a local hospital where he received sutures. Injury is classified as an ER-RWC.

While operating a trackhoe,employee had discomfort in his neck due to the machine movements.

On Saturday 2/14/09, EE slipped & fell while walking on trommel deck grating on Maxvill Wet Mill. EE was transported to a local hospital. EE sustained a torn meniscus & ligament as a result of the fall. Prescription medication was administered. Injury has been classified as an OER/RWC. Amendment: Reclassified as OER/LWC due to surgery on 3/11/09 (1st day missed work).

Employee stepped down from one level of grating to another. His leg landed awkwardly & felt a pain in knee. He received f/a & returned to work with no restrictions. The employee continued to experience discomfort & further medical evaluation took place. It was later determined that there was a slight tear in his meniscus & surgery was scheduled on 1/2/08 to repair the injury.

On Sunday, 9/25/05, an employee slipped and fell while walking on the bottom deck of the Trail Ridge Wet Mill. He was transported to a local hospital. The employee sustained two cracked ribs as a result of the fall. Prescription medication was administered. This injury has been classified as an ER-MTC.

Contact with electrical current

Employee checking on malfunctioning vibrator in a conveyor feed pit recieved an electrical shock. The employee developed numbness and limited mobility to his right arm.

Employee was using pneumatic blow-pipe to move spilled material under packaging assembly line, the material hit an obstruction and blew back into both eyes.

WHILE WORKING ON ROUGHER B HEAD FEED PUMP, THE PUMP SHEAVE BECAME DISLODGED AND FELL OFF THE SHAFT AND FELL OVER ONTO EMPLOYEE WHO SUSTAINED THREE SMALL CUTS FROM LINER OF HARD HAT. SUTURES WERE REQUIRED.

EE TRAVELING N. ON SR 301 HEARD NOISE & FELT SOME VIBRATION UNDER THE TRUCK. HE STOPPED TRUCK ONSIDE OF ROAD. AS HE EXITED THE TRUCK, FLAMES WERE COMING FROM UNDERNEATH. FLAMES WERE EXTINGUISH ED WITH NO FURTHER INCIDENT. HE WAS TRANSPORTED TO PLANT MEDICAL FOR CHECKUP. HE HAD QUARTER SIZED BLISTER ON TOP OF INSTEP, WHICH WAS CLASSIFIED AS A SECOND DEGREE BURN. OTC MEDICATION GIVEN

AFTER WORKING A 12-HOUR SHIFT, EE WITH PRE-EXISTING KNEE CONDITION WAS DESCENDING A FLIGHT OF STAIRS TO GO HOME. ABOUT 2/3 OF THE WAY DOWN THE STAIRS, THE INDIVIDUAL FELT A SHARP PAIN IN THE BAD KNEE WHICH CAUSED THE EE TO TWIST RIGHT ANKLE AND BREAK A SMALL BONE IN THE RIGHT FOOT.

WHILE IN THE PROCESS OF REMOVING THE DREDGE MOTOR BARREL ASSEBMLY, A COUPLING FELL AND PINCHED MECHANIC'S LITTLE FINGER ON THE LEFT HAND. SURGERY WAS REQUIRED TO REPAIR DAMAMGE.
