Customer was using 773 haul trucks to be loaded with boulders from LVP pit to be taken off site. During execution the trail haul truck lost brakes on a downhill slope between 6-10 degree decline. The trail truck struck the lead truck pushing it under LVP dozer trap. 911 called immediately and flight for life was dispatched. Front miner has broken ribs and back.

Showing all 4The operator was watering a road with a CAT 621G water pull. The operator was driving approximately 10 miles per hour when they drove into the equipment parking V-ditch. The operator stated that they knew it was there, but didn't notice it due to an optical illusion. The impact bounced them in the seat straining their lower back. The operator was having a hard time breathing.
Jaw feeder at plant primary plugged up and overflowed material. Miner was cleaning the jaw feeder underbelt with a shovel because the skid steer was tagged out due to mechanical failure. Miner was the ground person in the area and noticed the tail pulley was about to be buried. Miner decided to start clean up while belt and jaw were operating which is against company policy.
Miner failed to access a working area in a safe manner, resulting in a loss of arm.
