Customer was using 773 haul trucks to be loaded with boulders from LVP pit to be taken off site. During execution the trail haul truck lost brakes on a downhill slope between 6-10 degree decline. The trail truck struck the lead truck pushing it under LVP dozer trap. 911 called immediately and flight for life was dispatched. Front miner has broken ribs and back.
The operator was watering a road with a CAT 621G water pull. The operator was driving approximately 10 miles per hour when they drove into the equipment parking V-ditch. The operator stated that they knew it was there, but didn't notice it due to an optical illusion. The impact bounced them in the seat straining their lower back. The operator was having a hard time breathing.

HE FELL ONTO THE CONVEYER BELT, TRAVELING HALFWAY DOWN THE CONVEYER WHEN HE ROLLED OFF THE CONVEYER AND HITTING THE RIGHT SIDE OF HIS FACE ON THE FRAME OF THE CONVEYER, FRACTURING JAW DAMAGING EYE.
