Employee was going to wash off grease fittings. The employee was having an issue with the hose, so the operator was going to dismount the loader to help. The operator did not realize the loader was still in gear and as operator stepped off the brake pedal, the loader moved forward. The front right tire bumped the employee's knee.

Showing all 4Employee was backing up a dingo pushing dand when EE's hand was pinched in between the dingo controls and a cross member for the duster. There was light swelling and redness, treated as a first aid incident. When irritation persisted it was learned that the wrist was broken.
Employee operating skid steer when it suddenly pulled hard to one side causing employee to hit their head on side of window and later experiencing neck pain. Employee did not report incident and went to ER on their own.
The employee was removing a piece of equipment from the forklift to be worked on. The forklift was placed in a holding position but the emergency brake was not on and the controls were not touched when the forklift jerked forward striking the employee on the right side of body causing them to land on the connex box several feet away.
