Was traveling in shuttle car back to miner when a bent rib strap entered the kitchen striking EE on the back of the head. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
IE was a passenger in a mantrip that was travelling into a track spur. The mantrip struck a dolly that was unseen by the driver, and IE's back struck EE's seat hard. Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.
Employee was helping switch cars in belt spur when motorman #2 pushed cars towards EE. EE was standing near motor when EE's foot was injured. PPE was not a factor, mining systems was not an issue, compliance with rules and regulations was not an issue. Body placement was an issue.
Employee was operating mantrip dragging slick line when rope that was attached to pipe broke and piece of metal from rope struck EE's elbow. PPE was not a factor, mining systems was not a factor, compliance with rule and regulations was not a factor.

IE was travelling on the mainline track on a golf cart and encountered a motor trip travelling toward them. They attempted to get out of the way but was unable, and was struck by the motor.
Using FCT to pull cable slack with loops of cable tied to a drill steel placed on the FCT while in the 8 West belt entry. Injured was holding drill steel to keep the rope from pulling off, but the rope pulled off anyways and the drill steel came back and struck them in upper left leg resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
Employee was operating dump truck when it became stuck in mud. While attempting to get pushed out of mud by another piece of equipment, the dump truck moved or jolted causing employee to injure their neck. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was walking along stage loader when a rock struck EE injuring EE's finger. Mining systems was not an issue, compliance with rules and regulations was not an issue, PPE was an issue.
Employee pulling a power car in 7 West 3 left 3 block intersection by hooking a chain up between the power car and a scoop. They had scoop back up to take slack out of the chain and it failed immediately, throwing a link that struck them in the forehead resulting in 3 lacerations that required 6 sutures total to close. No problems with equipment issues, training or PPE.
D35 traveling toward Metz with clearance and collided with D84 (traveling towards St Leo without clearance) at 0 block 9 South causing injured to be thrown into the cab of the mantrip passenger compartment, resulting in 3 fractured ribs and 2 fractured vertebrae. No problem with equipment issues, training or PPE.

Moving passenger vehicle. The closing door struck the left side of EE's head, knocking head and most of body into the car. The EE received a minor cut on the R side of head, eyebrow area, but L leg, below the knee, was still outside the vehicle. The vehicle continued forward, still in contact with 12" barrier post, door closed, causing both bones in EE's lower leg to be fractured.
Employee was struck by a rock while working on 5 Belt at 29 1/2 wall, which resulted in a contusion to their left wrist. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
While applying a chain around a recently coupled supply car to a motor when the motor on the other side of the trip tried to uncouple, but the couplers got stuck and ended up pulling the supply cars and motor a short distance causing a strain to their shoulder and upper back. Started missing work 9/20/25. No problems with equipment or compliance issues, training or PPE.
Injured employee was driving a scoop to the section charging station when EE ran over a crib block causing EE to bounce up off of the seat. When employee landed back in the seat EE felt pain in EE's back and tail bone. Employee has training and experience.

Employee was hauling shields with motor, when motor came off track they struck against the kitchen resulting in bruising and strained muscles. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured operating 15 man personnel carrier down 9 South haulage between 90 and 120 block. Foot came off deadman switch, causing vehicle to go into neutral, injured looked down to switch vehicle back into direction and struck EE's hard hat off the mine roof. No problems with equipment or compliance issues, training or PPE.
While trying to uncouple an empty flat car that was off track from motor, they had the other motor pull on the car but it did not uncouple and shifted causing the coupler to swing and pinch left calf between the coupler and the motor resulting in a contusion. 64 block 9 north track. No problem with equipment issues, training or PPE.
Employee was operating a locomotive when EE hit EE's head on a cable bolt. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. EMPLOYEE DID NOT START MISSING TIME UNTIL JULY 15th.
Injured was hooking a chain onto a roll of belt inby 10 belt take up to be moved by the belt winder when another employee accidentally hit the lever of the belt winder causing the chain to tighten and catch the injured right pinky between the chain resulting in a distal tuft fracture. No problems with equipment, compliance issues, training or PPE.
While pulling fuel buffalo with 24 ton diesel motor through 118 1/4 block 9 south haulage they heard a noise like the buffalo went off track. Without stopping they turned back to look at the buffalo and struck their hard hat off a roof strap, resulting in an injury/aggravation to cervical spine. No problems with equipment or compliance issues, training or PPE

IE lifted the boom of a microtraxx machine. While IE reached in with right arm to check the boom pin, IE leaned on the remote box with chest, engaged the boom down function and lowered the boom onto IE's right arm. IE was trained as a microtraxx operator, but this was a compliance issue, as the machine was not de-energized, locked, tagged, or blocked against motion.
While operating golf cart going into 60# off 1 North haulage the injured struck hard hat off a wooden roof plank, causing pain in neck, upper back and shoulders. No problems with equipment or compliance issues, training or PPE.
Employee was in a mantrip was attempting to clear up off of the haulage, when the mantrip bumped into another mantrip that was in the spur. PPE was not a factor, mining system was not a factor, rules and regulations were not a factor.
Injured employee was riding on the outby end of a 7-man diesel mantrip. As the mantrip approached a track switch to get in, the mantrip operator failed to pay attention to the mantrip in front of them also entering the switch, and struck the end of that mantrip. The collision jarred the injured employee who complained of back pain which became worse during the shift.
Employee was operating a diesel locomotive on the outby end of a trip when they ran over a d-rail device causing them to d-rail and collide into the coal rib. Their right hand was caught between the locomotive body and rib. The result of the injury is amputation of the middle and ring fingers to the middle phalanges and amputation of the little finger to the distal phalange.
Employee was operating man trip when a piece of material got behind EE's glasses and entered EE's eye. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.
Employee was driving mantrip when they slowed down to stop, they hit their head on a strap. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was a factor.
Injured passenger in mantrip that collided with parked diesel dusters at 47 block St Leo. Some material fell out of the top and got into the mantrip operators eyes and when EE got their vision clear, EE was too close to dusters to stop in time. Injured broke right knee during collision. No problems with equipment or compliance issues, training or PPE.
Employee was pulling trip when motor derailed, employee was pushed into side of kitchen causing EE to injure EE's chest area. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Driving #9 Shuttle Car around turn at 98 blk #6 and kept turning into 5-6 crosscut, striking operator compartment against rib and dislodging a rib bolt strap into operator compartment striking EE's upper back and causing a contusion. No problems with equipment or compliance issues, training or PPE.
Employee stepped under a loaded muck bucket to clean the bottom when it dropped suddenly striking employee in the top of the hard hat.
Driving mantrip down St Leo haulage when EE encountered a rope hanger at 40 blk St Leo Haulage that had an S hook hanging on it which caught the hood of EE's sweatshirt tearing the hood and causing pain in EE's neck area. No problems with equipment or compliance issues, training or PPE. The rope hanger has been removed.
Employee was operating motor when EE was struck by a pipe that was along the track. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured was driving a shuttle car in the 7 West outby left ladder and ran over a rock in the tram road which caused injured to strike hard hat off the canopy which resulted in a strain/sprain to neck. No problems with equipment or compliance issues, training or PPE.
While riding in an open mantrip, a piece of mesh hanging from the roof struck employee in the left eye. This injury was not the result of a training or compliance issue.
Employee was passenger in a mantrip exiting the mine when they encountered a track switch left thrown for the turn. The caused the mantrip to enter the spur and strike a parked (vacant) mantrip. The impact resulted in the employee sustaining a sprained left ankle.

Employee was tramming a scoop. The EE was tramming scoop down entry when employee ran over roof strap causing a laceration on back of right shoulder. The cause of this incident was employee did not walk tram roads before operating scoop. IE was pushing a ram car up a hill with a tractor when EE got on the bumper of the car. When the tractor fell from the bumper, EE bounced into the canopy straining their back.
The EE stated: while driving a 4 man diesel EE ran upon a small gob pile, while backing up off the pile, the steering wheel spun around striking EE's left wrist
Employee was loading a loader onto a dolly and the loader slid off the dolly and cribbing. The impact from the loader bottoming out resulted in the slotted canopy to shift and pinched the employee against the controls of the loader. Compliance with rules/regs, PPE mining equip/systems not a factor. Miner proficiency may be a factor.
