turned 992 loader over into a pit. He was a contractor for Bates Contracting & Construction Inc. They reported the accident to MSHA. I got a violation issued on 7-9-09 to file an accident report on incident also.
Employee was riding bus (mantrip) and when it approached his piece of equipment he stood up as the bus operator braked and he fell and stuck his head on the header above the windshield. He was diagnoised with a cervical strain and concussion.
Employee was struck by ram car and suffered a laceration that required stitches.
Employee was operating a three wheel mantrip going down a slope, when he steered to make a turn the three wheeler turned over causing injury to his shoulder and neck. Due to complications he started missing work on February 05,2009.
Roof bolter operator was traming scoop and pulled into a crosscut to turn with his left hand on the scoop swivel bumper block outside the operators compartment. His middle, index and ring fingers were caught between the swivel block and were crushed. The middle and ring fingers were damaged to the base of his finger nails and had to be amputated at the base of the fingernails.
Employee alledges he was hurt on 7/24/2008 when he jumped off a mantrip. He began missing work on 8/5-2008.
Employee was operating a mantrip and steering wheel caugh and spun around catching his right hand. He suffered a fractured bone to his hand.
Employee was cleaning spillage in outside tunnel when a belt splice caught his clothing pulling him onto the #5 belt and he was then caught by the belt chute for the outside slate belt. He received contusions and lacerations to his upper back, chest, legs and arms.
Employee was struck by a chain in the back of the head that required stitches.
When preparing to enter the mine, the excellerator stuck on the buggy while in reverse. The 3 men on board had to evacuate the buggy as it was going to hit a concrete structure and possibly cause a serious or fatal injury.
Employee was backing a mantrip off of a steep place, when the wheels hit an off set in the mine floor and the mantrip turned over, causing a fracture to the lower left leg.
Employee reports that he was standing with his hands on the belt structure "looking up the belt line" when a belt splice caught his coverall sleeve. In order to free himself, he jerked with enough force that he dislocated his right shoulder.
Driver overturned loaded coal truck.
A loader ran into the deck of a three wheeled personnel carrier as it was being trammed through the stockpile area.
Employee was struck by a scoop and suffered a laceration to his left lower leg that required stitches.
While traming a scoop in the #4 entry the operator ran over a crib block and struck his head against the mine roof causing pain in his neck. He continued working and did not seek medical care until 1-15-07. He was diagnosed with a fractured vertabrae (C-4) and is off work effective 1-15-07.
Operator was in an A40 D Volvo truck while being loaded by a 988F Cat Loader. While the loader operator was preparing to dump the first bucket of rock in the truck bed a rock slipped out and struck the bed jarring the operator spraining his lower back.
While opening the airlock door near the mine drift, the scoop operator let the door he was attempting to open swing back catching his left little finger between the doors, lacerating it. Sutures were needed to close the wound he received. Medical Treatment Only-No days were lost.
Foreign body in left eye, cut eyelid.
Employee was tramming a personnel carriers through a air lock door and got caught betwen the carrier and top of door. First day missed from work 10/09/2006.
While operating a Joy 21 S.C., the injured had his right hand resting on the bottom of the operator deck & he caught his rt. middle finger between the deck & the pavement.
Employee was hooking up torches at #7 belt drive. He was between a discharge roller & a rubber tired mantrip (2-man). A scoop backed into the mantrip & he was caught between the mantrip & the discharge roller. He was transported to the hospital, kept overnight for observation & discharged with a diagnosis of bruised kidneys. Equipment inspected, no defects observed.
This individual was operating a 488 S&S scoop. He had pulled the rubber-tired supply car into a crosscut. The chain was hooked to the bucket of the scoop and frame of the car. He raised up the bucket to get slack in the chain to unhook it. This caused the car to drop and smash his right thumb between supply car and bucket.
The injured employee was operating a mobile bridge unit at the time the incident occurred. The canopy of the bridge unit caught a roof bolt and stopped suddenly, jolting the employee. (Employee notified foreman on 7/21/2005 of injury.)
Subject was attempting to back scoop around right corner and had his fingers in pinch point area and as a result his fingers were smashed but not broken.
Backing up, bridge operator left carrier to move miner cable. Light cord caught on left tram lever causing machine to swing sideways. Deck went on top of right foot.
SCOOP OPERATOR WAS CLEANING #6 ENTRY & LOST BRAKES ON SCOOP. RAN INTO COAL FACE & TWISTED BACK. BRAKES WERE WORKING PRIOR TO ACCIDENT.
