Electrician was changing the bearing on the #1 taipiece. When he went to grab the shaft a piece of metal went through his glove and stuck in his right index finger. When he removed it some was left in the wound. The metal was removed by the Dr. and (5) sutures were required. This is to report medical treatment only. No days were lost.
Unintentional roof fall 50' long x 20' wide x 3' to 5' in thickness. The fall is located on 8 left panel #14 crosscut and #8 entry.

An accumulation of methane was found from out gassing seals in a small isolated area. We consider this a non-inundation incident.

A fall has occurred 30' inby survey spad 1787 in the #1 entry of the No. 2 Southwest mains. Fall is approx 50' long X 20' wide X 5' high. Area has been timbered and dangered off. No affect to ventilation.
Employee began missing work this day because of a twisted back he suffered exiting a scoop. MRI indicated degenerative change and chronic no clinical.
This is to comply with violation #8075775 at the Josephine #3 Tunnel Mine. On 3/15/09 EE, 3rd shift Fire Boss, found loose rashy material had fallen from the highwall on the inby side of the fan house. The material measured approx. 9'H X 4'-6'L X 9'W.

Employee was in the process of changing rollers on the stacker belt when he caught his right hand between a roller and the belt rail causing a fracture to his right ring finger.

The injury was not reported until 11/23/08. We were informed that the EE was grinding metal and got something in his eye at that time.
EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.

While traveling airways a roof fall was identified in the old Mains Section at break #62 in the #1 entry near spad #358. The fall extended right handed into the crosscut and measured approx. 20'L X 25'W X 6'H. The area was supported with 5' torque tension and 12' supplemental supports. The area has been dangered off and timbers have been set.
