Pocahontas Coal Company LLC · Struck against stationary object

Electrician was changing the bearing on the #1 taipiece. When he went to grab the shaft a piece of metal went through his glove and stuck in his right index finger. When he removed it some was left in the wound. The metal was removed by the Dr. and (5) sutures were required. This is to report medical treatment only. No days were lost.

turned 992 loader over into a pit. He was a contractor for Bates Contracting & Construction Inc. They reported the accident to MSHA. I got a violation issued on 7-9-09 to file an accident report on incident also.

A fall has occurred 30' inby survey spad 1787 in the #1 entry of the No. 2 Southwest mains. Fall is approx 50' long X 20' wide X 5' high. Area has been timbered and dangered off. No affect to ventilation.

This is to comply with violation #8075775 at the Josephine #3 Tunnel Mine. On 3/15/09 EE, 3rd shift Fire Boss, found loose rashy material had fallen from the highwall on the inby side of the fan house. The material measured approx. 9'H X 4'-6'L X 9'W.

An unmapped mine was cut into while mining was being performed on the 001-0 MMU. A small amount of water was released into the active section. No Methane or low Oxygen was released from the old works and no hazards were noted. All Persons were withdrawn and production was immediately stopped.

The fall extended right handed into the crosscut and measured approx. 20'L X 25'W X 6'H. The area was supported with 5' torque tension and 12' supplemental supports. The area has been dangered off and timbers have been set.
