OPERATING MOTOR, COMING DOWN HILL, MOTOR STARTED TO SLIDE & STRUCK SCOOP CAR WITH TAIL PIECE IN BUCKET. HE WAS PINCHED BETWEEN TAIL PIECE ROLLER & MOTOR FRAME. MOTOR BRAKES WERE OPERABLE. FAIL URE TO FOLLOW AT A SAFE DISTANCE WAS CAUSE.
FRONT END LOADER OPERATOR PICKED UP BUCKET OF SHOT MATERIAL AND DUMPED IN ROCK TRUCK. THE BUCKET OF SHOT HAD A ROCK IN IT, WHICH THE LOADER OPERATOR DIDN'T KNOW WAS IN THE BUCKET; IT JARRED T HE TRUCK. HE DID NOT START LOSING TIME FROM WORK UNTIL 7/25/00.

HE STATED THAT WHILE RUNNING LOADER IN TIGHT SHOT, LOADER HIT BOTTOM AND JERKED HIM. BACK WAS STIFF AND SORE WITH PAIN RADIATING DOWN LEGS. HE DID NOT REPORT THIS UNTIL 5/9/00. HIS LAST DAY WO RKED WAS 5/5/00.

DISABLE BATTEY MANBUS WAS LODED ON LOWBOY, OTHER MOTOR COULD NOT PULL IT UP INCLINE. INJURED APPROACED TO ASSIST PUSHING IT UP INLCINE, THE MOTOR COUPLER WENT UNDER THE COUPLER OF THE MAN BUS CAUSING INJURED TOP PINCH FINGER BETWEEN FRAM EOF MOTOR AND FRAME OF MANBUS.
