EMPLOYEE BACKED THE ROCK TRUCK HE WAS OPERATING OFF THE NORTH DUMP, TRUCK TURNED UPSIDE DOWN & SLID APPROX. 75-100'TO THE BOTTOM. EE WAS WEARING HIS SEATBELT & REMAINED IN THE CAB. ANOTHER EE WAS MAINTAINING THE DUMP SITE & OBSERVED THE INCIDENT.

Showing all 5EE stopped on south entrance ramp to fuel pump generator. He shifted gear to neutral. Brakes wouldn't hold & truck rolled approx 100-125' down ramp toward highwall. He was afraid truck would hit wall & jumped from cab. Spoil material stopped truck as he jumped(temp was about 20 deg) When tested, brakes worked, however system was inspected, cleaned & 1 slack adjuster replaced.
Employee was operating rock truck co. #320 cat 777 when the top of the left front shock failed causing the truck to shake and bottom out aganist the frame. EE's left shoulder struck the side of cab. Left shock is located underneath the cab.
Employee was riding in the GI 6x6 truck when the driver ran over a rock causing him to bounce in the seat. He suffered a jar (strain) to his lower back.
ROCK FELL FROM WALL BROKE SIDE GLASS AND STRUCK OPERATOR ON LEFT ELBOW (LACERATION REQUIRED 6 SUTURES). LACERATION WAS APPROXIMATED 1 1/2".
