EMPLOYEE BACKED THE ROCK TRUCK HE WAS OPERATING OFF THE NORTH DUMP, TRUCK TURNED UPSIDE DOWN & SLID APPROX. 75-100'TO THE BOTTOM. EE WAS WEARING HIS SEATBELT & REMAINED IN THE CAB. ANOTHER EE WAS MAINTAINING THE DUMP SITE & OBSERVED THE INCIDENT.

Injured was dismounting the 500 Kamatsu loader. The handrails for the ladder were covered with frost. His left hand slipped off the handrail causing all his weight to bear on his right arm.

EMPLOYEE WAS MOUNTING THE 450 JOHN DEERE TRACK HOE. AS HE WAS CLIMBING UP HE STRUCK HIS HEAD ON THE OUTSIDE MIRROR BRACKET. HE JAMMED HIS NECK CAUSING NUMBNESS IN BOTH HANDS & RIGHT ARM.

Employee was changing the dust filters on the SKF Reed drill. One of the filters was stuck. When it came loose it fell on his right wrist causing his wrist to strike the frame of the dust filter housing.

Employee was operating rock truck co. #320 cat 777 when the top of the left front shock failed causing the truck to shake and bottom out aganist the frame. EE's left shoulder struck the side of cab. Left shock is located underneath the cab.

EMPLOYEE WAS PUSHING ROCK OFF THE NEW CASTLE SEAM. HE BACKED UP OVER A ROCK CAUSING HIM TO BUMP HIS ELBOW ON THE INSIDE OF THE CAB

THE LADDER USED FOR FUELING WAS MISSING & EMPLOYEE USED A BUCKET TO STAND ON. HE SLIPPED FROM THE BUCKET & SPRAINED HIS LEFT ANKLE, LADDER WAS PROVIDED. EMPLOYEE WAS RE-INSTRUCTED TO ALWAYS USE THE LADDER.

EE HAD HELPED THE MECHANIC REMOVE AN IDLER FROM A D-10 DOZER. HE WAS LOADING THE JACK INTO THE MECHANIC'S TRUCK WHEN THE JACK SLIPPED AND CAUGHT HIS FINGER BETWEEN THE JACK AND EDGE OF THE VEHICLE LICENSE PLATE. LACERATION TO RIGHT INDEX FINGER (REQUIRED STITCHES.)
