EE STATED HE HAD LOADED A CAR OF COAL IN #1 ENTRY AND TRAMMED HIS CAR 5 BREAKS AND HIT A HOLE IN THE ROADWAY 1 BREAK INBY THE FEEDER. HE FELT PAIN IN HIS BACK BUT CONTINUED TO WORK UNTIL THE M INER FINISHED THE CUT (APPROX. 15 MIN.) AND THE PAIN WAS TOO MUCH AND CAME OUTSIDE. THE PAIN WAS LOCATED IN THE THORACIC PART OF THE SPINE. RESULTED IN STRAIN TO THE BACK.
EE STATED THAT HE WAS AT THE FEEDER AND WAS GOING TO START THE PICK BREAKER FOR THE SCOOPY OPERATOR. AS HE REACHED OUT AND TOOK HOLD OF THE START BUTTON HE HEARD THE SCOOPY BEHIND HIM AND TWI STED AROUND AND FELT A POPPING AND BURNING IN HIS RIGHT SHOULDER.
INJURED STATED THAT HE WAS TRAMMING TOWARD THE FEEDER WITH A LOAD OF COAL, WHEN THE TRUNION BROKE ON THE DUMP END OF SHUTTLE CAR. THIS BOUNCED HIM IN THE DECK OF THE SHUTTLE CAR AND JARRED HI S LOWER BACK.
KEN STATED THAT HE WAS IN #3 ENTRY. THE HAD REPLACED PART OF SHUTTLE CAR AND THAT THE SLACK HAD BEEN PULLED OFF THE CAABLE REEL. HE HAD HIS RIGHT FOOT ON THE SLACK TO KEEP THE CABLE STRAIGHT . AS THE SLACK TOOK UP, THE CABLE PUSHED HIM UP AND INTO THE COAL RIB. RESULTING IN FRACTURE TO RIGHT LEG IN KNEE JOINT.
EE STATED THAT HE WAS ON HIS SHUTTLE CAR GETTING LOAD OF COAL IN 1 ENTRY THE MINER TAIL WAS AGAINST THE TOP LOADING COAL INTO HIS SHUTTLE CAR AND TRAMMING FORWARD HE FELT SOMETHING FO IN HIS L EFT EYE. A PIECE OF METAL WAS TAKEN OUT OF HIS LEFT EYE BY DR PRUITT AT THE DANIEL BOONE CLINIC IN HARLAN KY.
HE STATED THAT HE HAD GOTTEN INTO THE SCOOP BUCKET FOR MANTRIP FROM THE END OF THE TRACK TO THE SECTION. HE WAS ON THE OUTSIDE EDGE OF THE BUCKET. THE SCOOP WAS TRAMMING TOWARD THE SECTION WHE N THE SCOOP WENT THROUGH A SWAG AND THE SCOOP OPERATOR RAISED THE SCOOP BUCKET INTO THE ROOF, DRAGGING HIM ALONG THE ROOF FOR APPROX. 5 FEET BEFORE PULLING HIM OUT OF BUCKET INJURING HIS SHOU
INJURED EE WAS A PASSENGER IN A MANTRIP MOTOR THAT STRUCK A DERAILED MOTOR AND EE WAS THROWN ON HIS SHOULDER.
EE STATED THAT HE WAS OPERATING SHUTTLE CAR & WAS RETURNING TO THE MINER & AS HE WENT AROUND THE CORNER OF A BREAK THAT THE CANOPY BROKE LOOSE & FELL ON HIS HEAD RESULTED IN STRAIN TO BACK & B RUISED BACK
EE STATED THAT THE SCOOP WAS GOING UP #5 ENTRY CARRYING MANTRIP TO SECTION. HE WAS RIDING IN THE SCOOP BUCKET WHEN SCOOP BECAME STRUCK ON PILE OF LOOSE COAL, CAUSING SCOOP TO JUMP UP AND BOUNC ED HIM CAUSING LOWER BACK PAIN.
HE STATED THAT HE AND ANOTHER EMPLOYEE WERE OUTSIDE MAKING A BELT SPLICE AND WAS ON BOTH KNEES ON BOTTOM BELT WHEN THE #1 BELT STARTED AND THE BELT TENSION CARRIED HIM BACK AND CAUGHT HIM BETW EEN THE BELT AND BELT STRUCTURE.
WHILE RIDING IN MANTRIP TO SECTION FROM THE SURFACE, THE AUTOMATIC FIRE DELUGE SYSTEM ON THEPERSONNEL CARRIER DISCHARGED AND HE STATED THAT HE INHALED THE CHEMICAL RELEASE.
WHILE RIDING IN MANTRIP FROM SURFACE TO THE SECTION, THE AUTOMATIC FIRE SUPPRESSION SYSTEM ON THE PERSONNEL CARRIER DISCHARGED AND HE INHALED THE CHEMICAL RELEASE. DORAN WORKED 2 HOURS ON 3-32-93, THEN CAME OUTSIDE WITH SAME SYMPTOMS AND RETURNED TO DOCTOR.

STANDARD CAR WAS STUCK, TRYING TO PUSH IT OUT WITH OFF STANDARD CAR, INJURED WENT AROUND OFF STANDARD CAR OPERATOR DID NOT SEE HIM WHEN HE STARTED TO TRAM. STRIKING INJURED ON THE LEG AND RIG HT SIDE.

INJURED AND ANOTHER EMPLOYEE WERE RETURNING TO THE SURFACE COMING OUT OF MINE IN A RAILRUNNER.EELECTRIC BRAKES FAILED AND HYDRAULIC BRAKES WERE APPLIED,LOCKING UP WHEELS ON RAILRUNNER CAUSING IT TO JUMP THE TRACK ON SURFACE AREA.THE JOLT OFTHE WRECK CAUSED PAIN TO BACK AND RIGHT KNEE.INJURED WORKED EVERYDAY WITH PAIN UNTIL HE SAW A DOCTOR ON 05-28-92.INJURED HAD SEEN A DOCTOR ON 01
INJURED STATED THAT HE DROVE THE SCOOP TO WHERE OXYGEN AND ACTYLENE TANKS WERE STORED ON A SLED. HE SET THE BRAKE AND DROPPED THE SCOOP BUCKET. HE GOT OUT OF SCOOP AND LOADED HOSES AND GUAGE S FIRST. HE AND ANOTHER EMPLOYE EPULLED THE SLED UP IN THE SCOOP BUCKET WHENT EH SCOOP ROLLED FORWARD APPROXIMATELY 12 INCHES CATCHING HIS FOOT BETWEEN SCOOP BUCKET AND MINE FLOOR.

WHILE TRAMMING SHUTTLE CAR (STANDARD SIDE CAR) FROM FEEDER BACK TO CONTINUOUS MINER. TRAVELING ALONG THE #4 ENTRY ROADWAY, CAR TIRE DROPPED INTO A HOLE IN ROADWAY CAUSING INJURED TO BE BOUNCED UPWARD, THEN BACK DOWN SUDDENLY, RESULTING IN AN ALLEGED INJURY TO THE LOWER BACK.
INJURED STATED THAT WHILE TRAMMING TOWARD CONTINUOUS MINER TO LOAD SHUTTLE CAR, THAT CAR CROSSED OVER SOME TYPE OBJECT LYING IN ROADWAY WHICH IN TURN CAUSED A SUDDEN JAR RESULTING IN EE COMING UNSEATED THEN STRIKING RIGHT HIP ON DECK RAIL OF SHUTTLE CAR CAUSING INJURY TO RIGHT HIP LOWER RT SIDE OF BACK WITH NUMBNESS IN RT UPPER THIGH AND GROIN AREA.
WAS SAID TO BE RIDING IN ON MANTRIP ABRASION WAS CAUSED BY HEAD SCRAPING AGAINST MINE ROOF WAS WEARING HARD HAT & SAFETY GLASSES.
BELT WAS BEING PULLED IN AN ANGLE FROM MAIN LINE CONVEYOR WITH AN ENLOADER THE EMPLOYEE HAD POSITIONED HIMSELF IN AN AREA THAT WHEN BELTHUNG IT PULLED ACROSS STRIKING EMPLOYEE & KNOCKING HIM O FF A 6FT PLATFORM

INJURED WAS RIDING IN BACK OF PICK-UP, LOADED WITH STRAWBLOWER, THE STRAW BALES SHIFTED ALLEGEDLY KNOCKING HIM OFF LEFT SIDE REAR OF TRUCK, RESULTING IN LIGAMENTS BEING STRETCHED AND POSSIBLY TORN IN RIGHT KNEE.
THE OPERATOR WAS SITTING ON THE JOHN DEER BACKHOE NEAR THE RAILROAD TRACKS UNDERNEATH THE TRAIN LOADOUT WITH THE ENGIN RUNNING. A TRAIN, WHICH WAS LOADING AT ANOTHER LOADOUT, WAS BACKING UP A FAST RATE OF SPEED AND WITHOUT A BRAKEMAN. THE OPERATOR COULDN'T HEAR THE TRAIN UNTIL IT WAS ABOUT FIVE FEET AWAY. HE THEN JUMPED FROM THE MACHINE TO TEH GROUND. WE THEN TOOK HIM TO THE H

THE INJURED SAID HE WAS RIDING IN SCOOP BUCKET IN #4 ENTRY WHEN SCOOP HIT A LUMBER IN THE ROADWAY BOUNCING HIM UP AND WHEN HE CAME DOWN HARD,HURT HIS BACK AND LEFT LEG.
HE WAS OPERATING STANDARD SHUTTLE CAR IN ENTRY APPROX. 2 BREWKAS IN FEEDER GOING TOMINER AFTER A CAR OF COAL. HE HIT A HOLE IN THE ROADWAY, AND WAS BOUNCED OUT OF HIS SEAT AND WHEN HE CAME BA CK DOWN, HE LANDED ON HIS BACK ON OUTSIDE RAIL OF OPERATORS DECK.

WHILE DESCENDING HILL, FOR SOME UNKNOWN REASON, VEHICLE SUDDENLY ACCELERATED BEYOND SPEED DRIVERCOULD CONTROL. VEHICLE STRUCK BERM ADN FLIPPED OVER (TRUCK WAS LOADED) TRUCK HAS SEAT BELTS & THEY WERE NOT IN USE.
WAS DUMPING LOAD OF COAL AND WHEN BED ELEVATED THE HOIST PUSHED THROUGH THE BED OF TRUCK AND LOADED BED FELL BACK DOWN CAUSING A SEVERE BOUNCED AS BED STRUCK FRAME APPROX. 20 TONS WITH 4-5 FT DROP.

RAILRUNNER WAS 4RECKED ON SUPPLY YARD THE 988 B CAT LOADER WAS BROUGHT FROM RAW COAL PIT TO SET RAILRUNNER BACK ON TRACK WHILE SETTING RAILRUNNER OVER WHEEL BECAME LODGED AGAINST INSIDE OF RAI L WHEN LIFTED H3GHER WHEEL CLEARED RAIL SWANG OVER STRIKING INJURED ON RT HIP CAUSING DISLOCATION OF RT HIP JOINT
