EE WAS TRAVELING OUTBY ALONG BELTLINE WHEN HE STRUCK A POTHOLE, JARRING LOWER BACK. THIS ACCIDENT WAS NOT REPORTABLE UNTIL 10/19/01 AT WHICH TIME HE STARTED RECEIVING THERAPY.

WHEN NEARING THE PIT, HE APPLIED BRAKES AND WHEN DOING SO HE PUT THE CARRIER INTO A SPIN AND HIS LEFT HAND WAS ON THE SIDE RAIL WITH FIN GERS ON THE UNDER SIDE. THE RING FINGER APPARENTLY CAUGHT A ROCK BETWEEN BOTTOM AND UNDER CARRIEAGE OF PERSONNEL CARRIER. EE WAS PLACED ON RESTRICTED DUTY.
EE WAS PERFORMING THE REQUIRED PRE SHIFT EXAM & WHILE BACKING UP EE STRUCK HIS HEAD AGAINST THE ROOF. EE CONT WORKING UNTIL 1/8/01 AS HE WAS PLACED ON RESTRICTED DUTY AT THAT TIME
EMPLOYEE WAS APPLYING STONE ON TRAVELWAY WITH SCOOP WHEN SIDE OF SCOOP OPPOSITE THE OPERATORS COMPARTMENT SANK IN A SOFT SPOT, THUS PERMITTING THE OPERATORS SIDE TO RAISE AND IN DOING SO EMPLO YEE STRUCK HEAD AGAINST ROOF AS WELL AS A ROOF BOLT. EMPLOYEE RECEIVED STAPLES FOR LACERATION.
