Cat 777 haul truck was going up the ramp to the dump site, the truck started backwards made a turn and landed on its side. The driver said he could not remember what happened, it is probable that he dozed off, he did not attempt to hit the brakes for 75'. Driver had a few scrapes on his left arm, he was treated at the er and released to full duty with no restrictions.

EE STATED THAT HE WAS SUSING HIS CELL PHONE WHEN HE DROVE HIS PICKUP OVER A 30' HIGHWALL. EE HAD SMALL CUTS ON THE FACE, A LACERATION ON THE FOREHEAD AND A RUPTURED BLOOD VESSEL IN THE UPPER L EFT INNER THIGH.
EMPLOYEE HIT A BUMP IN THE HAUL ROAD EXITING THE PIT RAMP, WHICH HAD BEEN RECENTLY BACK DRUG FOLLOWING A RAIN EVENT. HE BOUNCED UP & HIT HIS HEAD ON THE ROOF OF THE CAB AND HIT HIS KNEE ON TH E WINDOW CRANK. HE DIDN'T REPORT THE ACCIDENT TO HIS SUPERVISOR UNTIL 8/25/00; HE STATED THAT EVERY DAY AFTER THE ACCIDENT, THE JARRING OF THE TRUCK MADE HIS BACK HURT MORE, & HE REPORTED THE
DRIVER HIT A BIG POT HOLE IN THE ROAD, WHICH JARRED HIM, CAUSING HIM TO HIT HIS HEAD ON THE ROOF OF THE CAB OF THE TRUCK HE WAS DRIVING. WHEN HIS HEAD HIT THE ROOF, IT ALSO JAMMED HIS NECK WIT H PRESSURE FROM THE HIT.
DRIVER WAS OPERATING THE TRACTOR TAILER AT A SPEED TO FAST TO SAFELY NEGOTIATE A CURVE ON THE PERMITTED TOWNSHIP ROAD, THEREFORE THE TRUCK/TRAILER TURNED OVER ONTO THE PASSENGER SIDE. THE JARR ING OF THE BODY INSIDE THE TRUCK RESULTED IN PAIN IN BACK, ANKLE, LEG, ARM BRUISES AND BUMPS.
