EE upset truck 147 and trailer 29 while dumping Bypro at the MeadZinn Tower cut. EE stated he was not injured to both several EE. EE was dumping bypro and had trailer up when the back third of the bypro broke loose and slid back and the rest stayed in the nose of the bed causing it to be top heavy and tip over.
Employee was using a heated power washer, he lost his balance and slipped and sprayed and burned his right foot leaving a second degree burn.
A D-ii operator noticed smoke under the cab of the D-11 operator working beside him. The operator of the D-11 that was smoking shut the machine down and with the help of the foreman tried to extinguish the smoke after about ten minutes the fire department was called because they could not get the fire out. The pit was evacuated at this time.
The truck driver went against company policy by climbing on his trailer to check for loose coal, as he was coming down the steps he lost his footing on the bottom step, he fell on the fender of the truck and then fell about 3 foot to the ground landing on his right hip.
Shop mechanic was using a hammer, he swung the hammer to strike the awe he was using, he missed the awe and hit his hand causing a small laceration on his left hand.
The water pumper stood on the tire of the water pump to check the fuel level and slipped off the tire and landed on his left elbow, bruising the elbow. He did not report the incident until 01/24/2011.
Worker was attempting to place a belly pan back on a Cat D-11 dozer, the pan was secured with chains on both sides, one chain shifted letting the pan shift backwards cutting the worker on the left ear lobe. Worker was taken for treatment which required 10-15 stitches and was released.
EE was working with a chain on the ground, when the EE's finger became caught, tearing skin from his finger tip. The EE's finger had gotten pinched between the chain hook and object he was attaching chain to.
Employee was located at the bottom of a highwall moving hose when a rock came loose and hit his finger.
EE's were setting new Ibeam support in plant. Ibeam was being supported by come-a-long and strap at top and was caught at bottom by 1/2"" plate. Ibeam stopped at top by another Ibeam, EE attempted to dislodge it by lifting on it in a crouched position and threw out his back. EE complained of pain and layed on floor. EE was placed on backboard and taken to hospital.
The employee was attempting to break tire bead on the right front tire of the front end loader with a porta-power cylinder positioned between the frame of the loader & the tire bead when the cylinder slipped out under pressure & struck the EE in the forehead.
EMPLOYEE WAS WELDING ON THE FINE COAL DRAG CHAIN AND RECEIVED AN ELECTRICAL SHOCK WHILE WELDING ON PINS OF 678 CHAIN. THE CAUSE OF THE ELECTRICAL SHOCK IS UNKNOWN. THERE WAS NOTHING EVIDENT WH ICH WOULD HAVE BEEN THE CAUSE.
EE WAS DRIVING DOWN A HILL AT MEAD ZIMM PIT WHEN HE LOST CONTROL AND LANDED IN A DITCH. EXCESSIVE SPEED WAS THE CAUSE OF THE ACCIDENT.
EE WAS OPENING THE TOP ROLLS ON GUNDLACH CRUSHERWHEN THE 1"" STEEL ROD (32"" LONG) SLIPPED & CAME OUT CAUSING HIS BODY WEIGHT TO SHIFT WHICH CAUSED HIS RIGHT ANKLE TO TWIST, BREAKING IT.
EE STATED THAT HE WAS SUSING HIS CELL PHONE WHEN HE DROVE HIS PICKUP OVER A 30' HIGHWALL. EE HAD SMALL CUTS ON THE FACE, A LACERATION ON THE FOREHEAD AND A RUPTURED BLOOD VESSEL IN THE UPPER L EFT INNER THIGH.
EMPLOYEE HIT A BUMP IN THE HAUL ROAD EXITING THE PIT RAMP, WHICH HAD BEEN RECENTLY BACK DRUG FOLLOWING A RAIN EVENT. HE BOUNCED UP & HIT HIS HEAD ON THE ROOF OF THE CAB AND HIT HIS KNEE ON TH E WINDOW CRANK. HE DIDN'T REPORT THE ACCIDENT TO HIS SUPERVISOR UNTIL 8/25/00; HE STATED THAT EVERY DAY AFTER THE ACCIDENT, THE JARRING OF THE TRUCK MADE HIS BACK HURT MORE, & HE REPORTED THE
DRIVER HIT A BIG POT HOLE IN THE ROAD, WHICH JARRED HIM, CAUSING HIM TO HIT HIS HEAD ON THE ROOF OF THE CAB OF THE TRUCK HE WAS DRIVING. WHEN HIS HEAD HIT THE ROOF, IT ALSO JAMMED HIS NECK WIT H PRESSURE FROM THE HIT.
TWO CAT LOADERS WERE IN A #4A COAL PIT AT THE MEAD/ZINN NORTH PIT OF THE BOWMAN STRIP OPERATION LOADING COAL. IT HAD RAINED HARD THE NIGHT BEFORE AND HAD RAINED EARLIER IN THIS SHIFT. A SECTIO N OF THE HIGHWALL FELL AND HIT ONE OF THE LOADERS BELOW, THE ROCK HIT MAINLY THE LEGFT SIDE AND FRONT OF THE LOADER--SOME OF THE ROCK CAME THROUGH THE FRONT WINDESHIELD, HITTING THE OPERATOR.
DRIVER WAS OPERATING THE TRACTOR TAILER AT A SPEED TO FAST TO SAFELY NEGOTIATE A CURVE ON THE PERMITTED TOWNSHIP ROAD, THEREFORE THE TRUCK/TRAILER TURNED OVER ONTO THE PASSENGER SIDE. THE JARR ING OF THE BODY INSIDE THE TRUCK RESULTED IN PAIN IN BACK, ANKLE, LEG, ARM BRUISES AND BUMPS.
EE WAS OPERATING CO. NO. 520 D7 DOZER, ANOTHER EE WORKING WITH HIM CALLED THE SUPERVISOR AND REPORTED THE HE WAS SICK. EE COMPLAINED THAT HIS SHOULDER, NECK AND RIGHT ARM FELT NUMBNESS IN THEM ; HE STATED HE FELT LIKE HE WAS GOING TO FAINT. DR. AT HOSPITAL RAN TESTS AND XRAYS, NO PROBLEMS WERE DIAGNOSED, BUT EE IS TO RETURN TO DR. HE NORMALLY USES 3-2-2000.
