Employee was traveling in a mantrip when he hit a block causing him to bounce around in mantrip. Employee sustained a fracture to his back and will remain off work for an uncertain period of time.
Injured was pinned between the movable roller and the take-up frame after putting a come-along on the roller to free it. The roller broke free and struck the injured knocking him to the ground and proceeding to run over him causing severe injuries.
Injured was operating a scoop, when he got out of the scoop his back begin to hurt. He does not recall a specific incident that may have caused his back to hurt.

Employee was driving a mantrip when he turned to look back and unintentionally decelerated, when this happened a mantrip following behind hit the back of employees ride jolting employees back. At the time of the incident employee didn't take any medical treatment. Employee is now having pain and is seeking medical treatment.

Employee was a passenger in a mantrip traveling out. While backing up the mantrip ran into rib and hit employees arm against the rib. Employee went to Good Samaritan Hospital sustaining a contusion to his arm. Doctors ordered to remain off work for an uncertain period of time.
Employee was operating miner, checking his sites before cutting when he was struck by a hauler amputating his leg. Co-workers applied tourniquet, treated for shock and cared for him while traveling out of the mine to the helicopter where he was flown to U of L for treatment.

Employee thought he could work but came out before the end of shift wanting to go to the doctor. Employee sustained a neck strain/sprain and was ordered to remain off work for an uncertain period of time.

Employee was walking back to equipment when hauler approached intersection slidding in the mud and hitting the scoop inwhich struck employee into rib pinning him between canopy and rib. Employee was Lifeflighted to Deaconess Downtown; treated for a fractured pelvis and shoulder, ruptured ear drum and artery. Employee will be off work for an uncertain period of time.

Employee was a passenger in a Genco mantrip, when traveling up the mine slope out of the mine, when they collided with another Genco mantrip. Employee was struck against an unknown object, resulting in a strain/sprain to his back. Employee went the next day to Dr. where he received treatment.

THE EMPLOYEE WAS A PASSENGER IN A MANTRIP WHICH CAME IN CONTACT WITH THE COAL RIB. THE EMPLOYEE'S HEAD STRUCK THE INTERIOR FRONT PLATE OF THE MANTRIP. THE EMPLOYEE WAS TRANSPORTED TO THE DEACONNES HOSPITAL VIA LIFE FLIGHT. HE REMAINS IN THE HOSPITAL AT THIS TIME.
EE trammed minitrac in front of feeder and into crosscut, cal hauler padded under curtain while ringing bell. Minitrac & coal hauler contacted on another neither piece of equipment was damaged and pie pans did not fall off of forks.
Employee was backing up mantrip when he hit parked scoop. The employee struck the left brow on the canopy post, resulting in a laceration that required 3 sutures.
Employee was driving in diesel mantrip when she drove through bulk dust that had been placed on the roadway. The rockdust flew up & obscured vision. She hit the coal pillar with the vehicle. She sustained a cracked orbital socket on the right side.
EE was operating shuttle car, he hit coal rib and coal flew off pillar and entered his left eye.
The employee was standing between the mini-trak and the trash trailer unloading trash when the mini-trak operator tammed forward onto the employee's right ankle. The victim sustained a fractured right ankle.
Employee was out of shuttle car @ feeder when 2nd car came to feeder and hit employee's car. Employee sustained a bruised left knee.
Employee was tramming diesel scoop along 1 North roadway when he ran over rock in roadway & jammed neck into canopy. Employee continued to work until taking off beginning on September 2, 2008 due to pain. This accident became reportable on 09/02/08 due to missing work.
Farm tractor rollover.
EE was attempting to relieve shuttle car driver, when he was struck by shuttle car and knocked down. EE missed work on 5/10/07.
Employee was behind line curtain on #3 unit in the 4 right face when shuttle car leaving continuous miner ran over left foot.
Employee was operating Getman Diesel Tractor & pulled into crosscut to turn around. He had his right ring finger out of the cab in articulation area. As articulation closed, his finger was mashed just past the last knuckle requiring sutures.
Employee was carrying a seal block to the 2nd North #4 seal when he lost his footing, slipped & fell, landing on his rear on a timber. He re-aggravated the injury the next day when he was operating his shuttle car, hit a bump in the roadway & hit his head on the canopy of the shuttle car.
EMPLOYEE KNELT DOWN LEANING AGAINST THE RIB FILLING OUT HIS MAP, WHEN THE SECTION SCOOP OPERATOR CAME FROM ENTRY #1 TO ENTRY #2. WHEN THE SCOOP CAME THROUGH THE CROSSCUT THE OPERATOR TURNED THE CORNER HEADING OUTBY & THE BATTERIES ON THE SCOOP STRUCK THE EMPLOYEE IN THE CHEST & LEFT ANKLE.
EMPLOYEE WAS GOING TO WORK ON HAWKEYE AT THE SECTION TAILPIECE. HE SHUT BELT OFF AT TAILPIECE. BEFORE THE TAILPIECE QUIT ROTATING, HE REACHED IN TO PERFORM WORK. HE CAME INTO CONTACT WITH THE TAILPIECE WITH HIS LEFT MIDDLE FINGER. THE LEFT MIDDLE FINGER SUSTAINED A FRACTURE.
EE PARKED MANTRIP JUST INSIDE CURTAIN AND WAS LOADING OIL CANS INTO SHUTTLE CAR WHEN MINITRAC CAME THROUGH CURTAIN AND BUMPED MANTRIP AGAINST EE'S RIGHT ANKLE CAUSING A SPRAIN.
AS EMPLOYEE WAS RIDING IN MANTRIP, VEHICLE HIT BUMP IN ROADWAY. EMPLOYEE FLEW UP FROM SEAT AND LANDED ON RIGHT TESTICLE. EMPLOYEE WORKED FOR ANOTHER WEEK BEFORE REPORTING INJURY.
A SIDE RAIL OFF OF A CONTINUOUS MINER WAS BEING DRAGGED OUT OF THE WAY WHEN THE RAIL CAUGHT ON A TIRE AND FLIPPED AROUND STRIKING EE ON RIGHT KNEE. EE SUSTAINED A CONTUSION TO THE RIGHT KNEE.
EE JUMPED ONTO REAR BUMPER OF MAINTENANCE VEHICLE, AS THE VEHICLE WAS BACKED THROUGH VENTILATION CURTAIN IT STRUCK THE REAR END OF A PARKED SCOOP. THE COLLISION RESULTED IN THE RIGHT LOWER LEG BEING FRACTURED BETWEEN THE KNEE & ANKLE.
