EE was at section feeder running rock through feeder when he attempted to hit steel bar to activate e-stop. The steel bar slid out by and did not hit e-stop. EE hit e-stop cable which caused a rock to flip over and pinned his hand and cut it.

WHILE RIDING MANTRIP, EMPLOYEE WAS FACING OTHER WAY THAN TRAVEL WHEN HE TURNED AROUND HIS HEAD CAME IN CONTACT WITH OVERCAST, LACERATION OF FOREHEAD.
Employee was operating a scoop on the roadway on 1st right parallel with A belt and hit a hole in the roadway. He jammed his head and neck into the canopy, having pain in neck on the right side.

POSSIBLE BROKEN NOSE, CUT ON UPPER LEFT SIDE OF LIP AND RED SWOLLEN SPOT ON CHEEK BONE BELOW LEFT EYE. FOUR BREAKS INBY #3 HEAD ON TRACK ENTRY AT AIRLOCK DOOR.
EMPLOYEE WAS LEAVING MINE SITE AND WAS TRAVELING ON HAUL ROAD. HE LOST CONTROL OF HIS VEHICLE, STRUCK LEFT SIDE FRONT FENDER INTO HILLSIDE AND VEERED RIGHT ACROSS ROAD RUNNING VEHICLE OVER THE EMBANKMENT INTO A TREE. EMPLOYEE WAS OFF DUTY - LEAVING WORK.

SCOOP OPERATOR ROOFED HIMSELF WHILE CLEANING SECTION. SWOLLEN SPOT ON RIGHT SIDE OF HEAD. TRANSPORTED BY AMBULANCE TO THE HOSPITAL. EE WAS TREATED AND RELEASED SHORTLY AFTER EXAM.

EMPLOYEE WAS OPERATING #1 MANTRIP WITH HIS LEFT LEG CROSSED UNDER HIS RIGHT LEG AND HANGING OUTSIDE THE MANTRIP AND AS HE TRAMMED OUTSIDE HIS LEG WAS CAUGHT BETWEEN THE MANTRIP AND HALFHEADERS THAT WERE PILED BETWEEN THE BELT AND TRACK.

WHEN EMPLOYEE PUSHED LEVER FOR SCOOP TO MAKE A R IGHT TURN, BATTERY END OF SCOOP COMES AROUND ANDHITS UP AGAINST RUBBER STOP THAT HE HAD HIS LEFT HAND ON, MASHING TIP OF MIDDLE FINGER LEFT HAND.
EE WAS OPERATING A SCOOP IN LOW COAL, LESS THAN 42" HIGH, NO CANOPY ON SCOOP. WHILE PASSING THROUGH UNDER A MINER CABLE THAT WAS HUNG FROM MINE ROOF, EE GOT HIS HEAD PUSHED BACK BY THE CABLE C AUSING PAIN IN NECK.
EE WAS HOLDING A ROCK DUSTING HOSE WHILE ROCK DUSTING STANDING IN FRONT OF SCOOP. SCOOP WAS BACKING THRU A FLY CURTAIN AND NOTICED THIS BREAK THRU WAS BLOCKED BY ANOTHER PIECE OF EQUIPEMTN. SC OOP OPERATOR THEN POST SCOOP IN FORWARD TRAM POSITION AND TRAMMED FORWARD SEVERAL FEET. INJURED EE DID NOT KNEW THIS AND KEPT WALKING BACKWARDS WHILE DUSTING. SCOOP BLADE HIT INJURED EE OR BA
