Showing all 8EE was shoveling at the end of the Finishing Screen. A rock fell, hitting him on the right shoulder. He reported it to his supervisor. He said he was ok and went back to work. On 2/15/07, one hour before work time, he called in saying he was going to the doctor because his shoulder was hurting. The doctor excused him from work for 2 days.
Employee was going to the pit to haul shot rock. We had a lot of rain the day before. The road had been washed out underneath. The front of the truck dropped in the hole making a sudden stop. Employee hit her knees on the dash.
IW was stndng on the lft side of the trlr. The drvr chked the side mirror, he saw IW begin to walk away from the trlr. The drvr then noticed a pu truck approaching on the rt side. The drvr looked to see that the pu was not pulng in front of him. He then began to pull fwd. While pulng fwd he rechckd his mirrors and saw IW falling to the grnd. The drvr stopped the truck.
Employee was mounting the shaft and gear reducer on the belt winding equipment. He was using the front end loader with a chain to hold the gear reducer and the chain slipped off of the edge of the bucket.
EE WAS DRIVING STOCKPILE TRUCK AT #1 PLANT WHEN COMPLAINED OF LOWER BACK PAIN. HE BELIEVES THAT MOTION/RIDE OF TRUCK CAUSED HIM TO STRAIN LOWER BACK. EE HAD SEEN A DOCTOR BUT NO CONCLUSIVE RES ULTS WERE MADE. ON 6-4-02 EE CALLED IN & SAID THAT HE WOULD NOT BE IN (BACK WAS HURTING) & GOING TO DR'S OFFICE. DR SAYS HAS ABNORMALITY CALLED SPONDYLOLYSIS. MISSED 1 DAY.
EE WAS REMOVING THE TAIL GUARD ON M-74. UPON REMOVAL, THE GUARD GOT HUNG UP. EE CLIMBED UP ON THE BELT TO FREE IT AND WHEN HE WENT TO THROW IT OFF IT CAUGHT HIS COVERALLS. IT FLIPPED HIM OVER AND HE HIT AGAINST THE TAIL SECTION OF THE CONVEYOR AND THE GROUND.
EE WAS JOGGING A CONVEYOR BELT FORWARD TO MAKE A BELT SPLICE. A PIECE OF SAND WENT IN HIS EYE. EE WAS WEARING HIS SAFETY GLASSES.
