Employee lifted bucket of oil to set on loader said pulled back. Told supervisor they lifted it wrong. MD wrote it is their medical opinion that employee may return to work on 05/17/2022. Employee was seen at medical group,
Employee said a rock struck their thumb while greasing. Was seen at hospital in and put on light duty for 3 days. When employee was questioned on 4/20/2022 about conflicting statements, employee resigned.

Injured employee was standing under Jaw Crusher another employee yelled and when injured employee moved quickly Hat fell off and a steel plate struck employee on top of head. Injured employee received 4 staples to close wound.

On 2/15/07, one hour before work time, he called in saying he was going to the doctor because his shoulder was hurting. The doctor excused him from work for 2 days.
