Material fell on front end of loader and hit windshield, causing windshield to break and laceration to operator left and right arm. Highwall is about 25 to 30 feet high.
EE STOPPED HIS HAUL TRUCK TO TALK TO ANOTHER DRIVER. HE LEFT TRUCK IN GEAR AND WHILE HE WAS STANDING ON HIS CATWALK, HIS HAUL TRUCK ROLLED INTO THE OTHER HAUL TRUCK. EMPLOYEE WAS MAKING REPAIRS ON AN AIR CONDITION-ING SYSTEM ON A LOADER. HE THOUGHT HE'D TURNED THE DISCONNECT TO THE OFF POSITION; HIS KNEE HITTHE REMOTE START WHILE MAKING THE REPAIRS & THE ENGINE STARTED.
EE RAN INTO THE BACK OF A PAN TRK (CONVERTED INTO WATER WAGON) WITH A 55TON HAUL UNIT. WHEN HE REARENDED THE PAN, THE HAUL UNIT BOUNCED CAUSINGA MUSCLE STRAIN IN THE LOWER BACK IT APPEARS THAT HE FELL ASLEEP (CONVERSATION WITH WIFE RE VEALED HE HAD BEEN UP ALL NIGHT THE NIGHT BEFOREWITH A SICK CHILD)
