EE was climbing a step ladder when EE's foot slipped and EE missed a step. To keep from falling, the EE grabbed on to a piece of machinery to steady themself. EE said that they felt like they may have pulled a muscle. The pain got worse over the next few days.

Showing 50 of 59CMI Maintenance Team Member was wiring an electric vibration device to a new hopper when a co-worker approached the team member to ask a question. Some time later the injured team member complained of back & leg pain & indicated they were going to the hospital for medical attention. Injured complained of back pain for 3 weeks from an old injury prior to this.
Miner step on a hoe which came forward and struck them in the left elbow. Miner complains of soreness of the left elbow.
The EE was descending a ladder, lost grip and fell. The EE sustained injuries to the left arm (laceration and possible broken arm, laceration to the right thumb).
Employee suffered from a headache due to smoke inhalation while putting out a small fire.
EE was removing a bearing from a trunion when developed an allergic reaction. Doctor could not determine the cause of the reaction, suggesting an insect bite. EE was performing a routine job that EE has performed many times, so it cannot be a reaction caused by the bearing, otherwise it would happen weekly.
EE was moving some boards and got a splinter in EE's right hand. Between thumb and fore finger.
EE's were putting the hood on the crusher. Co-worker was operating the track hoe & lifted the hood up to crusher. When the hood got on top of crusher, it got stuck sideways & the chains came off. IE climbed up on the frame of the crusher & re-attached chains to the hood. IE tried to move hood from side to side. The hood popped up & hit IE's leg knocking IE 6' to the ground.
EE was loading a tanker truck from silo. Had opened the hatches using fall protection, then came down to load truck. After truck was loaded, EE filled out the paperwork & gave it to driver. As usual, EE then climbed back up the truck to close the hatches. EE turned to tie off using fall protection, but truck started driving away. EE grabbed an I-beam to hang on to.
Employee was bitten by black widow spider while walking to office building; bite was on right of neck causing swollen area size of quarter at bite area; He was transported to emergency room for treatment and released same day. Spider was confirmed to be black widow by emergency care providers.
While lifting 25HP electric motor from mount the lifting eye on top of motor broke and dropped 2' hitting mount and rolling off mount onto employee's foot. Lifting eye was installed by manufacturer. Steel choker & clevis were used for rigging. Truck hoe was used for lift.
Dredge oper. was in the bottom of dredge checking the pump when he lost his footing on his right leg. When he came down on his right leg his knee was twisted or sprang.
Stepped off dredge to frozen ground. Rolled ankle over, bad sprain.
Attempting to lift sheet of metal stepped off sidewalk onto another piece of metal, twisting his knee.
Around 8:00 pm the loader was going to the pit to put lights on. Went to shop to fuel loader bucket was bouncing. Then the loader stopped. EE had no seat belt on. He hit the windshield with his forehead and nose.
Operator was mining wall throughout the day, When about 4:20 pm operator was backing out from wall when wall collaspe. Material fell on front end of loader and hit windshield, causing windshield to break and laceration to operator left and right arm. Highwall is about 25 to 30 feet high.
Employee stepped from scales to deck to clock in and in doing so turned his foot causing pain in his lower back.
While using a side grinder employee was struck in the eye with a small piece of shrapnel. The extent of the injury was minimal.
EE WAS LEAVING BOILER THRU CHAIN CONVEYOR. HE STEPPED FROM TOP LEVEL ONTO BOTTOM SCAFFOLD BAORD. THE BOARD BROKE AND HE FELL 6 FEET INTO CHAIN CONVEYOR.
EE LOST HIS FOOTING WALKING ALONG RAILROAD TRESSELL GOING TO HIS CAR.
EMPLOYEE WAS GETTING INTO LOADER TO FILL ROCK BIN. HE LIFTED RIGHT LEG TOO HIGH TO REACH FIRST STEP, BUMPED RIGHT KNEE ON SECOND STEP OF LOADER.PAIN WAS SO BAD HE PASSED OUT FALLING INTO FLOOR OF LOADER. HE BUMPED HIS HEAD AND RIGHT SIDE AS HE FELL.
EE WAS SWEEPING, REMOVING DIRT FROM UNDER TABLE WITH A BROOM, WHEN HE FELT SOMETHING IN HIS EYE.
EE WAS PREPARING A BOILER TUBE TO REPAIR A CRACK IN THE TUBE. AIR FROM THE PNEUMATIC GRINDER BLEW DUST UP UNDER HIS WELDING HOOD.
EE WAS STEPPING OVER PIPE & CAUGHT HIS FOOT & FELL LANDING ON HIS RIGHT HAND. EE WAS DISTRACTED & WAS NOT PAYING ATTENTION TO HIS FOOTING.
EE WAS MOVING A 450# DRUM OF OIL ACROSS SHOP FLOOR WHEN HE CAME TO A NARROW OPEING BETWEEN THE 4160 ROOM, DRUM CART ROLLED OVER A SEAM ON THE FLOOR. DRUM CART HIT THE SEAM IN THE FLOOR & JARRE D THE CATCH LOOSE FROM THE DRUM, ALLOWING DRUM TO MOVE UP & FORWARD.
EE STEPPED ON A PIECE OF AROMA FLEX (BLOCK FROM INSULATION), THAT WAS PARTLY UNDER WATER AND HADMUD ON IT. EE SLIPPED AND FELL ON HIS RT. HIP, WHEN HE FELL HE HIT THE LOCKER ALONG THE POWER HOUSE AND WALL.
KNEELING ON CONCRETE, SCRAPING GASKET SURFACES. IMPROPER KNEELING & BENDING PROCEDURES.
EE WAS INSIDE CATIOR VESSEL ON PT #1 TAKING FINGERS OFF DISTRIBUTION READER FOR REPAIRS. AS HE WAS PUSHING HIS WAY OUT OF THE VESSEL, RE CUT HIS LEFT HAND ON THE STAINLESS STEEL DISTRICUTION H EADER.
HAND WAS IN PRONE POSITION WHEN PIN WAS PULLED HAND WAS PINCHED IN BETWEEN A PINCH POINT IN MOWER.
EMPLOYEES WERE CUTTING AND WELDING PATCHES ON SEWAGE TREATMENT PLANT.
WORKING ON GATHERING SCREW AT E-40 BAGHOUSE. CONTRIBUTING FACTOR WAS HEAT.
THE AREA WHERE EE FELL WAS SLIPPERY WITH ALGAE GROWING, BECAUSE OF A STOPPED UP DRAIN.
KNOCKER WRENCH FELL FROM LEDGE OF EXPADSHION JOINT AND DOWN ONTO TOP OF FOOT.
DO NOT KNOW EXACTLY EE FELT PAIN WHEN HE WAS WALKING DOWN FRON E-40 BAGHOUSE.
WORKING ON FLY ASH VALVE 141 HAD END COVER OFF AND WAS APPLYING AIR PRESSURE TO VALVE WHEN VALE PISTON FLEW OUT HITTING RIGHT HAND AND CUTTING RIGHT THUMB.
EMPLOYEE WAS STANDING ON THE FENDER OF THE LOADER WASHING THE WINDOW. HE WAS HOLDING ON TO SIDE MIRROR WITH ONE HAND. HE LOST HIS BALANCE AND FELL TO THE GROUND. HE WAS COMPLAINING OF A HURT B ACK AND SORE LEG.
DRILLING HOLE AT AN ANGLE WHEN DRILL LOCKED UP AND CAUGHT EE'S HAND BETWEEN STEEL.
CLIMBING DOWN BETWEEN BOILER AND CATWALK. EE PUT HIS KNEE ON A ROUND PIECE OF SKIM CAUSING TO GET HIS FOOTING BELOW WHEN HE FELT HIS KNEE POP.
LIFTING CHUTE OUT OF MILL WHEN RIGGING WAS BINDING. LET OFF RIGGING TO ADJUST STRAP CHUTE WAS WEDGED BUT WHEN ADJUSTING STRAP IT FELL ON HAND.
TRYING TO LINE UP SPRINGS IN PULVERIZER BY TURNING COUPLING. HE FELT SHARP PAIN IN CHEST. HE AT THAT TIME DID NOT REPORT ACCIDENT BECAUSE HE DID NOT THINK IT WAS THAT BAD.
TAKING CENTER POST FROM D COAL MILL & CENTER POST FELL FROM MILL LANDING ON UPPER CHEST
EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE WHO WAS MOVING A WELDING MACHINE. THE INJURED EMPLOYEE WAS CARRYING THE MAJORITY OF THE LOAD. THE INJURED EMPLOYEE FELT A PAIN IN HIS GROIN AREA.
EMPLOYEE WAS USING A PIPE WRENCH & STRAINED HIS BACK. EMPLOYEE DID NOT REPORT INCIDENT TO SUPERVISOR UNTIL 7/25/00 & DID NOT REQUEST TREATMENT UNTIL 7/28/00.
EE WAS REMOVING A HOIST FRICTION HE DID NOT WEAR GLOVES & WAS CUT RESULTING IN 6 STITCHES
USING A WRENCH TO TIGHTEN BOLTS. THE WRENCH SLIPPED, CUTTING HIS RIGHT RING FINGER.
EE WAS COMING OUT OF A SPACE & THE PERSON ABOVE HIM DID NOT SEE HIM THERE. THE PERSON ABOVE OPENED THE BUILER DOOR AND HOT GLY ASH FELL ONTO THE INJURED EE'S NECK & SMALL SECTION OF HIS RIGHT ARM.
EE WENT TO GET THE ATTENTION OF A CONTRACTOR. THE CONTRACTOR TURNED AROUND WHILE UNHOOKING A GAINT GUN AND EE WAS SPRAYED WITH PAINT.
EE STOPPED HIS HAUL TRUCK TO TALK TO ANOTHER DRIVER. HE LEFT TRUCK IN GEAR AND WHILE HE WAS STANDING ON HIS CATWALK, HIS HAUL TRUCK ROLLED INTO THE OTHER HAUL TRUCK. HE WAS PINNED AND GRABBED ONTO THE BED OF THE OTHER HAUL TRUCK, DROPPED TO THE GROUND, RAN AFTER HIS HAUL TRUCK, CLIMBED UP AND STOPPED HIS HAUL TRUCK. HIS INJURIED OCCURRED AT IMPACT.
EMPLOYEE WAS MAKING REPAIRS ON AN AIR CONDITION-ING SYSTEM ON A LOADER. HE THOUGHT HE'D TURNED THE DISCONNECT TO THE OFF POSITION; HIS KNEE HITTHE REMOTE START WHILE MAKING THE REPAIRS & THE ENGINE STARTED.
EMPLOYEE WAS REMOVING BOLTS TO CHANGE OIL ON AN ARTICULATING TRUCK AND WAS HOLDING UP THE BELLY PAN WHEN HE LET THE PAN DROP, HE DIDN'T MOVE HIS HAND OUT OF THE WAY ON TIME.
