EE states was cleaning a conveyor table with a T-rake, the T-rake got into the roller pulling EE's hand into the roller. EE went to the ER, X-rays were taken and all was negative and no restrictions. EE went back to our work comp clinic on May 17th and was given restrictions to also start physical therapy on May 22nd.

EMPLOYEE WAS UNPLUGGING RAW SHALE HOLE WITH PIECE OF PIPE. SHALE STARTED TO FALL DOWN AND HIS LEFT FOOT GOT COVERED, SHALE FEEDER BELT WAS TURNED ON AND HE WAS SUCKED DOWN TO SHOULDER DEEP.
