Employee was helping repair a conveyor belt that was torn in two different areas. After the first area was repaired, the employee was rolling the V-Belts to advance the belt to a better position for repair when employee's left hand was pulled into the V-Belt sheave, almost completely severing the thumb.

Driving haul truck down a road, lost control of truck and rolled it 1 time down the road. Rolled over 1 time. Bed landed on wheels, cab rolled 1 1/4 times and came to rest w/driver's side on ground. Driver was wearing seatbelt and sustained very minor injuries.

EE NWAS DRIVING ON OFF HIGHWAY TRUCK STOCKPILING GRAVEL. HE GO TOO CLOSE TO THE EDGE AND TIPPED THE TRUCK OVER. HE WAS THROWN AROUND IN THE CAB PRETTY HARD. HE STARTED COUGHING UP BLOOD AND TA STING BLOOD IN HIS MOUTH SINCE THEN. HE QUIT WORKING FOR US THE FOLLOWING WEEK.
