992C LOADER DUMPED BIG ROCK FROM BUCKET INTO TRUCK. TRUCK BOUNCED FROM IPACT CAUSING EMPLOYEE TO BOUNCE. THIS ACTION STRAINED EMPLOYEES NECK.

I.NAME WAS SITTING IN A 35 TON HAUL TRUCK WITH WINDOW OPEN AND FOREARM RESTING ON WINDOW TRACK LOADER WAS PUTTING 2ND BUCKET ON TRUCK WHEN A 5 IN TO 6 IN ROCK FELL OFF THE LOADER BUCKET AND BRUISED LEFT WRIST AND TOP OF HAND
EMPLOYEE WAS LOADING HAUL TRUCKS THAT WERE STOCK PILING. BACKED UNDER CONVEYOR AS THEY REVERSED IT.
EMPLOYEE ATTEMPTED TO SHUT LOADER ENGINE OFF USING STANDARD PROCEDURE(PULLING ACCELERATOR PEDAL UP)WHE HIS HAND SLIPPED OFF OF PEDAL THUS STRIKING THE BOTTOM OF THE DASH.THIS IN TURN RESULTED IN A LARGE "BUMP" RISING ON THE BACK OF HIS HAND OVER THE FIRST KNUCKLE-DOCTOR'S DIAGNOSIS-A BRUISE.

THE EMPLOYEE WA8 HELPING TO INSTALL A 16" DIAMETER PIPE ON THE 16" PUMP. HE WAS PUTTING A 1" BOLT THROUGH THE FLANGE OF 16" PUMPP AND THE FLANGE OF 16" PIPE. THE PIPE WAS BEING PLACED BY A FRO NT-END LOADER. 9MPLOYEE HELD THE BOLT AGAINST THE PIPE FLANGE. THE LOADER OPERATOR MOVED THE PIPE CAUSING IT TO SHIFT BACK TOWARD THE PUMP, THEREBY PUSHING THE BOLT AGAINST THE PUMP CAUSING M
EMPLOYEE WAS CL9ANING OUT A RETURN ROLLER. BELT CAUGHT HIS LOOSE CLOTHING AND PULLED HIS HAND OVER TOP OF OUTSIDE ROLLER.

EMPLOYEE WAS DR3VING EUCLID W1 AND REACHED THE TOP OF HILL TO DUMP LOAD. THE ENGINE QUIT AND VECHILE STARTED TO RUN BACKWARDS. EMPLOYEE APPLIED FOOT BRAKE AND EMERGENCY BRAKE. VEHILE DID NOT S OP, TURNED INTO3BURN. EMPLOYEE JUMPED FROM VEHICLE, LANDING ON RIGHT FOOT. FRACTURED JOINT ABOVE LITTLE TOE RIGHT FOOT.
