Employee was changing VSI parts. EE was using a hammer to pound on parts, to loosen them up for removal. As EE was doing this, EE got a small piece of steel in EE's right eye. EE went to an ophthalmologist to have it removed. Employee was wearing safety glasses.

Employee was using a battery powered impact wrench to tighten a a bolt that was head high. The wrench slipped off the bolt and kicked back, hitting EE in the mouth. EE split lower lip open and chipped a front tooth. EE sought treatment. A dentist repaired the tooth that same day. EE did not receive any stitches in the lip.

Employee was using a pressure washer to clean debris from underneath a truck scale. While doing this, the nozzle slipped, causing an abrasion on the back of the employee's right hand, similar to a burn. Employee sought treatment. The hand was wrapped and employee was given an antibiotic.

Sometime in January (employee is unsure of date), employee was lifting a steel beam. EE picked it up and put it on EE's shoulder. EE felt pain in EE's left shoulder. The pain persisted for several weeks, but EE did not report the injury until April. EE had EE's shoulder checked, then had an MRI. It appears EE will need surgery.

Employee was using a sledge hammer to pound a nut on the mantle of a coarse crusher. EE felt something in the right elbow. It resulted in swelling and loss in range of motion.

Employee was walking, slipped on ice. Theye twisted right knee. EE didn't want to see doctor. It still hurt on Feb. 13 so they visited doctor. They told them to ice it, wear compression sleeve. EE went for follow up on Feb. 20. It still bothered them, so was sent for x-rays. EE has another followup on Feb. 27. Should have results and more information at that time.

Employee was working on an auger; EE slipped and hit EE's right knee on the auger's edge. EE finished the work day and went home. Later EE's knee swelled up, was bruised. EE went to the doctor the next morning. EE got an X-ray, which didn't show anything. EE returned to work, without any restrictions, limitations, prescriptions, or further treatment.

Employee was using an air compressor with an air sprayer to clean out the cab of truck. When employee was on way home that night, right eye started to bother. Went to the doctor the next day, and the doctor removed a foreign object from eye.

The EE was changing a screen panel on processing plant. He threw the old panel over the plant hand rail. The panel fell back. It hit the EE, knocking his hard hat off & hitting him on the right side of his head near his temple. The EE finished his shift, but felt dizzy. The next day, he still felt dizzy, so he went to the doctor. The doctor determined the EE suffered a concussion.
Employee was using an iron bar to hammer in the skirt rubber on a conveyor. He missed or slipped, and hit his right hand on the edge of the hopper box. He broke the pinky finger on his right hand.

EE was working in a van, roll-up door was open part way. Was climbing in. Part of body & one leg were in,& was pushing himself the rest of the way in. Foot slipped, & scraped his right shin on the edge. Continued working. That night, noticed a big bruise on his leg, and it was swollen. The next day, it was worse, so he went tot the ER. He had a strep infection in his skin.

EE was cleaning out scale. As he was putting the top doors back on, he felt a ""pop"" in his left elbow, followed by pain and cramping in his left bicep.
Employee was repairing a primary crusher, when he strained some muscles in his groin. He saw the company doctor, who diagnosed the injury as a hernia. He was referred to a surgeon. NOTE: The employee did not report the injury to the employer until Sept 19

EE was changing screens when he pulled some muscles in his stomach area. He waited a couple of days to see if it would improve. It didn't, so he went to see the doctor on July 26th. He was diagnosed with an umbilical hernia. It is not restricting him, so he decided to wait until this fall to have it reassessed and to schedule surgery.

Employee dropped a pipe wrench on his hand, 4 days later he told his foreman he had a hard spot on his wrist.
Employee was using a cutting torch to cut material. Apparently, the connection was loose where the torch and hose came together. This caused the fitting to leak a little. The torch ignited, burning the employee's left hand.

Employee was welding off and on all day on a jaw crusher, doing repairs. He was wearing a welding helmet, but he was ina tight spot, so it was difficult to get the mask down in time, every time, before he started welding. He had welding flash as a result.
