While operating 773 haul truck (equip# 1391) with a load, EE was driving down the Phase-2 haul road between the L-1 bench and H-3 bridge, the truck hit a bump in the haul road and EE tried to brace himself with his legs to cushion the bump. In that process he strained his inner right thigh. EE reported incident at the end of his shift on 1/28/13

While in a skid steer loader traveling down an inclined roadway with bucket up and filled with material, employee felt that the loader was going to tip forward. Employee reacted by trying to brace himself in the equipment and accidently hit the control stick for the bucket. The bucket came down and smashed his left leg, above the ankle, into the equipment.

While working in Phase II, employee was operating the bench loader, moving material from the face of the wall to bring a boulder down. The boulder slid, dropped and hit the front bucket of the loader, causing the rear of the loader to lift 3' to 4' off the ground. Upon landing employee felt pain to his back and neck. Employee reported incident at the end of the shift.

On 7/1/09, employee called supervisor to report that he was not coming in due to lower back pains. Employee thinks that while operating the dump loader on 6/30/09, the jerking action while driving and hauling materials from Phase II to the Primary Crushing unit, is what caused his back to be sore.

While operating the 2754 Loader in phase 1 moving secondary rocks on the floor, employee drove the loader into the face of the wall and felt pain in his back. He thought that the pain would go away but it didn't. Reported incident on 4/10/08.

Around two weeks prior to June 1, 2007, EE claims to have suffered a lower back strain from the jerking of the body in a sitting position while operating a defective loader over a period of time. Allegedly the controls to operate the bucket is defective thus causing more constant use of throttles and pedals. Employee reported injury on June 12, 2007.

While holding safety latch open on the compressor #3068, a truck backing up made contact with the compressor, smashing employee's left pointer finger between Track Bar and Tow Bar.

While operating loader #2745 on uneven ground, back and right side ribcage area began to hurt while trying to hold self in position on the seat. The loader would stop suddenly at full turn of the steering wheel.

While operating the CAT 773 off-highway truck #1391 at the bottom of the quarry haul road, EE drove over a bump in the road that allegedly tossed him around in the cab. He struck his right hand on the gear shifter handle. He continued to drive until his palm started to swell.

EE had the bed of his truck tilted up and hit the overpass with the corner of the dump. The jolt pushed his right arm back into his shoulder while steering the truck. EE felt a little discomfort but waited to see if he could shake it off. As a result of the impact EE strained R arm and shoulder.

While approaching the H-3 underpass from Phase II in truck #1389, EE locked on his breaks when he noticed our service truck standing by to let him pass. When employee locked on his brakes it caused the truck to bounce up and down, thus aggravating his back.

While Truck 1390 was parked at the Primary Plant to dump its load, hydraulic oil caught fire and EE jumped from the truck to the ground. When EE hit ground, he reports that he hurt his back.
