Employee was operating a machine that fills polypropylene bags with pulverized limestone when his left hand got caught in one of the straps attached to the bag, as the forklift operator was backing out with the pallet that the bag was sitting on resulting in the employee's left arm being pulled causing a strain to the machine operator's left shoulder.

Showing all 13while working on the belt line, employee was picking up a bag of dust when another bag fell striking his hand and pushed it into the belt causing an avulsion of the nail right hand ring finger.
Employee disregarded safety proceedures and labels by staying on forklift and attempting to release reclaim bin lever instead of dismounting and releasing the lever. The safety cage and fork assembly came down trapping right arm between cage and release lever.
Operator speeding on forklift, hit concrete slab, swerved, overcorrected and forklift went up on side at 35 degree angle and came to rest against pallet of product.
Forklift flipped over and cage may have caught employees foot. Employee also told that he hurt his foot at home when truck ran over it.

Employee was stacking dust - had knee against plate/platform. Platform was hit with endloader, causing plate to strike right knee.
