While lifting angle iron with a telehandler, a fork caught on a welded piece of metal on the side of the scale platform. When the weld broke, the fork sprang upward, jolting the angle iron upward, which struck the mechanic's jaw. First aid was applied, but on 5/21/25 EE reported ongoing pain. On 5/27/25, a CT scan revealed a closed fracture of the right maxilla.

Showing all 22An employee was assisting in loading a roll of a conveyor belt into the back of a service truck. The employee was standing in the back of the service truck. As the roll was coming off the forks of the telehandler, it gained more momentum than planned and the roll pushed the employee's lower legs against the air tank reservoir behind them.
Possible seizure truck bounced EE hit head on dash board area.
A rock came off of a conveyor and hit employee on left arm. Arm was bruised.
Employee was operating loader and backed away from backside of feed hopper, with bucket in a raised position. Traveled approx. 28 feet away from hopper with the bucket raised and backed up on unleveled ground causing loader to flip on it's side.
EMPLOYEE COMPLAINED OF PAIN RADIATING FROM HIS GROIN DOWN HIS LEG AFTER OPERATING A SCRAPER.
Employee was operating a scraper when he ran the scraper over uneven ground causing him to be jostled. Employee was diagnosed with a fractured lumbar spine. Subsequent investigation revealed that cushion hitch had not been activated.
Incident reported on 5/2/05, stating that lower back started hurting & pain increased thru the week. A new seat was ordered & installed. Restrictions issued by doctor on 5/2/05 did not inhibit him from performing his regular job duties as a plant operator but our company safety director contacted MSHA in Colorado on 5/19/05, and it was determined to be reportable to MSHA.
WORN SEAT ON FRONT END LOADER
Loader seat suspension caused upper back strain.
WHILE OPERATING FRONT END LOADER, EE WAS EXPERIENCING SEVERE DISCOMFORT AND PAIN IN HIS UPPER & LOWER BACK. HE STATED THAT HE WAS ABLE TO CONTROL IT TEMPORARILY BY ROUTINELY GETTING OUT OF THE LOADER & STRETCHING & WALKING. EE IS SCHEDULED TO SEE A DR. FOR A RE-CHECK ON 8/21/03 & AN MRI IS ALSO SCHEDULED. HE IS ON RESTRICTED JOB DUTIES UNTIL FURTHER NOTICE.
WHILE TURNING HIS FRONT END LOADER, EE FELT SOMETHING POP IN IS LEFT SHOULDER. AFTER GOING TO THE OCCUPATIONAL HEALTH CENTER, HE WAS PUT ON RESTRICTED DUTY. HE IS NOT ALLOWED TO STEER THE LOAD ER SO HE IS TEMP. ASSISTING ANOTHER EE WITH MAINTENANCE TYPE WORK.
EE REPORTED TO HIS SUPERVISOR THAT THE PREVIOUS AFTERNOON HE HAD BACKED HIS FRONT END LOADER INTO A BIG BOULDER. WHEN THIS HAPPENED, THE LOADER CAME TO A SUDDEN STOP. EE WAS FEELING STIFF ON 1 -15-03 SO HE WENT TO THE CLINIC WHERE THEY PUT HIN ON RESTRICTED DUTY.
EE WAS OPERATING A FRONT-END LOADER IN THE PIT AREA, WHEN HE DROVE OVER A WIND ROW MADE BY THE DOZER, WHICH JARRED HIS BACK AND STRAINED IT. INJURY WAS NOT REPORTED TO SUPERVISOR UNTIL 10/14/ 02. EE WENT TO THE OCC. HEALTH CENTER & THEY PUT HIM ON RESTRICTED DUTY AS OF 10/14.

BOUNCING ACTION OF LOADER CAUSED SHARP PAIN IN LEFT SIDE AND NECK.

EMPLOYEE WAS CHIPPING A WELD AND FELT A TWITCH IN HIS RIGHT ELBOW. THREE HRS. LATER, WHILE OPERATING A BOBCAT, HIS RIGHT ELBOW HURT HIM WHEN HE TRIED TO STRAIGHTEN OUT HIS ARM.
EMPLOYEE DIDN'T REPORT INCIDENT UNTIL 8-10 OR 8-11 WHILE CLAIMING HE INJURED BACK 7-29 WHILE WORKING IN LOADER.
