Struck by flying object

Employee was on the wash plant main screen deck cleaning off the walkway when a rock bounced out of a nearby in-feed drop box and struck the employees left hand.

Fall to the walkway or working surface

Employee was exiting a motor grader, EE was facing the steps of the grader while holding onto both handles. As EE was completing the dismount to the ground, EE's right foot slipped on uneven surface, and EE twisted right ankle which caused EE to fall.

Struck against stationary object

Employee was performing EE's pre-shift inspection on Loader 944K2 and went into the lube shed to get a few rags to clean the windows on the loader. As EE exited the building and was walking back to EE's loader, EE struck EE's right shin on a large set of wheel chocks that were stacked together in front of the building.

The employee went to start the pump on the water truck with the pull rope and the handle broke cutting EE's right index finger, which required 6 stitches.

Over-exertion in lifting objects

Employee was shoveling/moving wet raw material under a conveyor belt to the walkway just to the employee's right in the wash plant surge tunnel, when employee felt some discomfort in left shoulder blade. This was a first aid until May 14th when employee had initial visit at the clinic.

Employee was replacing shoes in the VSI crusher with new ones. During this task their back started to feel tight. Later that day this employee went to install a scraper on a conveyor and as they stepped over a beam they felt a twinge in their back.

Employee was stepping off the D8D7 dozer and as employee reached the ground with left foot first, employee rotated to the left and felt a pop in left knee. The ground was just graded flat.

Fall from machine

Employee stated that as he was exiting the truck his foot slipped off the step and he landed on the ground in an awkward maner. He later stated that his knee was sore. He was taken to the clinic as a pre caution where it was diagnosed as a minor strain. Return to work regular job.

Employee was in Scale office before shift. Stood up from chair to walk to fax/copier, took one step and knee gave out. (Did not hit anything, twist or turn). Employee said she had been exercising a lot using ""WII Fitness"" program.

Employee came to work with a brace on right wrist- said he'd been to his doctor-told he has carpal tunnel. Our HR dept sent him to a company referred doctor on 9/20 at which time he was given Ibuprofen (800 mg) 30 tablets & put on work restrictions which make him unable to perform his regular duties as a loader oper. He is currently working as a plant oper.

Installing belt scraper on stacker head pulley. Pipe of scraper barely in mounting fork, went to slide collar into place in gear box, caused pipe to fall off fork. Trying to keep scraper from falling, grabbed pipe too close & the force of it falling pinched his hand between the pipe & mounting fork.

EE FELT A PAIN IN THE CENTER OF HIS CHEST AS HE WAS LIFTING A LARGE ROCK OUT OF THE SCREEN CHUTE. EE DID NOT GO TO THE CLINIC UNTIL 6-7-02 AT WHICH TIME HE WAS GIVEN RESTRICTIONS AND A PRESCRI PTION FOR 400MG. OF ETODOLAC.
