EE was operating the shuttle wagon while loading cement rail cars. EE had EE's head out the window, watching for signals from the ground man. A gust of wind, blew some debris up and under EE's safety glasses and into EE's right eye.
EE was conducting a preventative maintenance check on some belt lines. EE reported a trough roller had a hole in it. EE said EE used a pry bar to check for the hole in the roller. During this check, EE's sleeve was unbuttoned, and EE's sleeve was caught by the roller and belt and pulled EE's arm between the conveyor belt and a trough roller. EE pulled pull cord to free self
Cleaning area with forklift, employee attempted to lift forks over the divider, forks were not able to lift high enough resulting in the fork to drop back down smashing employee's left ring finger.
Miner was operating a front end loader and sustained a contusion of lower back when the seat of the loader bottomed out when miner hit a bump. Miner was placed on restricted duty upon medical review of the injury.
Miner was stockpiling material away from the mortar sand stacker belt when the 988K loader EE was using came in contact with the belt. The belt was pulled onto the loader and the head pulley came thru the front windshield of the loader as it fell. Miner injured EE's left foot when the dash of the loader compressed onto EE's foot. Miner had surgery on foot to correct the injury.
Miner had a personal injury that miner came to work with and while operating the M982 loader it irritated the injury more. Additional strain of right lower back muscle due to improper posture while operating the loader.
Keller Electrical employee was directing coworker who was driving a pickup truck. Coworker turned the wheel of the truck and the tire drove up the right leg of employee causing employee's right fibula to fracture and other lower right leg injuries.
Failure to control the vehicle, he struck a berm, causing injury to his face.
While cleaning under the belt a rock bounced of the conveyor and struck him on the wrist.
When he opened the hood on the 980H the fan blew something into his eye.
While he was mucking out the kiln the ramp collapsed beneath him and caused the uniloader to drop. at the time of the accident the employee did not feel the need for medical attention until 3/27/07.
He stuck his hand into an opened inspection port to see if he could feel air passing through. His finger was cut.
Spotting bulk truck using sample spoon to align boot when boot started into trailer. Driver let clutch out and the truck lurched forward pulling the boot and sample spoon straining the employees wrist.
He was trying to stop a noise of an idler on the conveyor belt and his arm was pulled over the idler between the idler and the belt. This employee declined medical attention at the time of his injury, until Nov 6 he asked to see a Doctor for this injury.
He was removing a tractor protection valve at the rear of the #3 Dart, when removing one of the air lines, air rushed out and hit him in the face, knocking off his glasses and sent particles into his eye.
He was operating the skid steer and ran over a rock causing sharp motion and injuring his back.

While two vehicles were about to hit he tried to stop one of them. The boom truck was pulling a truck that was stuck, when the truck came down off the berm he tried to stop it and his foot got stuck in between them.
Dust fell off the inside of cab onto EE & into his eye. While moving piles of dirt in the storage area. Eye was flushed out & EE went home & came back to regular shift on the next day, he worked 1/2 his shift & then wanted to see a Dr. It was cleaned out and EE returned to work.
CAT TRUCK WAS TRAVELING TO B-6 PIT. THE TRUCK SLID OUT OF CONTROL ON A MUDDY, ICY CURVE. THE 35T WATER TRUCK WAS COMING DOWN THE HILL FROM THE SAME PIT. THE HAUL TRUCK SLID SIDEWAYS, THE BED OF THE HAUL TRUCK STRUCK THE CAB OF THE WATER TRUCK.
HE WAS RUNNING THE LOADER, MOVING MUD, BUILDING UP THE DIKE ON THE POND, RUNNING OVER ROUGH TERRAIN BOUNCING & JARRING HIS BACK.
EE WAS MOVING A STACKER W/ TRANSFER CAR- HE CHECKED TO MAKE SURE THE STACKER ARM WAS CLEARING THE RECLAIMER BOOM AND POSITIONED HIS FOOT OFF THETRANSFER CAR WHEN HE PASSED THE RAIL HIS FOOT GO T PINCHED.
AS HE WAS CLEANING THE CLAM SHELL ON THE FEEDER THE CONVEYOR BELT CAUGHT THE PRY BAR AND TWISTED IT OVER CATCHING HIS THUMB.
WHILE DIGGING INTO A HIGH WALL THE BANK GAVE WAYAND BURIED THE LOADER WITH MATERIAL CAUSING INJURY TO HIS BACK.
UNLOADING LIME AT PHX CEMENT. CAP ON BOTTOM OF TRAILER BLEW OFF,BLOWING LIME DUST INTO THE EYES AND THROAT.
WHILE SHOVELING @ THE DUMP POINT ONTO BELT CONVEYOR 202, EE'S FOOTING SLIPPED IN FINE MATERIAL. WHILE TRYING TO CATCH HIMSELF FROM FALLING, HE THREW HIS ARMS UP, AND WHEN HE DID, HE CAUGHT HIS GLOVE IN AN IDLER AND PINCHED HIS FINGER.
DRIVING ON ROUGH ROAD EE FEELS HE HURT HIS BACK-PRE-EXITING CONDITION PRESENT MAY NOT BE WORK RELATED-PEDING INVESTIGATION.
HE CAME UP THE WALL IN THE PIT TO GET A BUCKET OF MATERIAL./ HE WAS IN THE WALL ABOUT 3 OR 4 FEET HIGH. SUDDENLY THE WALL FELL DOWN AROUND HIM.
WHILE DOING C/U WORK ALONG CONVEYOR BELT, THE WINDS CAME UP. WIND BLEW THE BELT OFF IT'S IDLERS, HITTING HIM AND FORCING HIM INTO THE RAIL FOUNDATION. BELT WAS RUNNING, CAUSING BURNS & SCRAPE.
AS HE WAS REMOVING THE LEFT HOIST CYLINDER FROM THE FRONT-END LOADER, HE HAD THE BUCKET UP SITTING ON TOP OF A HOPPER. HE STARTED TO REMOVE OIL LINE UNDERNEATH THE MACHING WHEN SUDDENLY THE BU CKET FELL. THE LOADER ROLLED BACK AND AS HE TRIED TO GET OUT FROM UNDERNEATH HE TRIED TO GET OUT FROM UNDERNEATH HE TWISTED HIS ARM AND HURT HIS LEFT HAND.
HE WASWIDENING A ROAD WITHA FRONT-END LOADER WHEN SUDDENLY HE BOUNCED OVER A ROCK THIS MOTION CAUSED HIS INJURY.
WHILE DRIVING A QUAD HE LOOKED OVER TO HIS RIGHT SIDE TO CHECK HIS WORK, THE BIKE HIT A BUMP AND THREW HIM OFF CAUSING HIS INJURIES.
EE WAS WORKING IN AN UNAUTHORIZED AREA. HE PUT HIS ARM BETWEEN TWO ROLLERS, TO CLEAN ONE OF THEM OFF. THE ROLLER CAUGHT THE SLEEVE ON HIS JACKET TWISTING AND PULLING OFF HIS ARM AT THE ELBOW.
