Employee was cleaning near the Pan Conveyor at 731-B East side. While removing material from the walkway, the employee slipped on uneven ground, and the left hand contacted the moving conveyor, which lacked proper guarding. Onsite personnel assisted and transported the employee to a medical facility for stitches on two fingers.

employee was burned when hot kiln feed dust discharged from the tertiary air drop-out box discharge chute when the employee started to remove the collection dumpster under the chute. The material flushed over the dumpster and onto the forktruck and the operator, causing the injuries and damaging the forktruck.

Raw Material Handler was performing equipment check on a Komatsu loader and was checking belt tension in engine area and did not realize engine was still running when his middle and ring finger on his left hand became caught in V-belt. The ends on both finger were amputated just below the finger nails.

HE STATES THAT AS HE WAS DOING THIS HE SUDDENLY FELT SOMETHING IN HIS LEFT EYE. HE WAS WEARING HIS SAFETY GLASSES AND SUSPECTS THE DUST FELL OFF HIS EYE LASHES OR CAME FROM SOME SWEAT RUNNING INTO HIS EYE. HE FLUSHED HIS EYE TWICE

WORKER WAS STANDING BESIDE AN ELECTRIC MOTOR SUSPENDED BY A 2' WIRE ROPE CHOKER ATTACHED TO T-HEAD BOLT LAID ACROSS FORKS OF FORKLIFT. DRIVER BACKED INTO UNSEEN CHUCKHOLE CAUSING MOTOR TO SWIN G BACK & FORTH WHILE 8-10" OFF THE GROUND. THE FORWARD MOTION CAUSED THE BOLT TO SLIDE OFF THE TIPPED FORKS & THE MOTOR DROPPED ONTO HIS LEFT BOOT TOE.
