RIDING ON BACK OF ELE MOTOR WHEN TRAIN DERAILED SLAMMING EMPLOYEE INTO BACK OF MOTOR.
EE DISMOUNTED A FLATCAR WHILE IT WAS STILL IN MOTION. WHEN HIS LEFT FOOT HIT THE GROUND HE HEARD A "POP" AND FELT PAIN IN HIS KNEE. THIS INCIDENT RESULTED IN A LEFT KNEE STRAIN CAUSING EE TO L OSE TIME FROM WORK.
HIT HOLE ON HAUL ROAD WITH SCRAPPER CAUSING HIM TO HIT WINDOW HANDLE WITH FACE-CUT TO RIGHT CHEEK (TWO STITCHES)
EMPLOYEE AND LOCOMOTIVE OPERATOR WERE CARRYING 4' X 6' X 5' LAGGING ON TOP OF LOCI. THE LAGGING CAUGHT ON A CABLE FORCING IT UP TO THE OPERATOR'S CAB STRIKING EMPLOYEE'S RIGHT HAND WHICH WAS R ESTING ON THE CONTROL BOX. CUTTING RIGHT MIDDLE, RING AND PINKY.
PRIOR TO BLASTING (TO INITIATE A TURNOUT) THIS MINER WAS HELPING REMOVE AN ARCH (LOWER RIB) SEGMENT. AFTER BEIGN CUT FREE WITH A TORCH THESEGMENTS ARE THEN PULLED OTU SING A WIRE ROPE ATTACHE D TO A 30 TON LOCI. THIS MINER'S SUPERVISOR TOLD HIM TO MOVE TO A POSITION OF SAFETY IN CASE THE WIRE ROPE BROKE. WHILE THEMINER MOVED, IT WAS NOT FAR ENOUGH. THE ROPE BROKE BRUISING HIS TH
EMPLOYEE WAS RIDING IN A MANTRAIN.AS THE MANTRAIN PROCEEDED THROUGH THE AIR DOORS THE FIRST CAR CAUGHT THE DOOR CAUSING THE MANTRAIN TO COME TO A SUUDEN STOP.THIS CAUSED EMPLOYEE TO BE KNOCKED OFF THE BENCH AND STRIKING HIS KNEE AGAINST THEBENCH BRACKET.THIS INCIDENT RESULTED A LEFT KNEE STRAIN CAUSING EMPLOYEE TO LOSE TIME FROM WORK.
WHILE WALKING FROM THE TIME OFFICE TO THE WAREHOUSE A VEHICLE PASSED THE EMPLOYEE THIS VECHICLE STIRRED UP DUST & PARTICLE WENT INTO HIS EYE THIS INJURY RESULTED IN AN EMBEDDED FOREIGN BODY W ITH A RUST RING LEFT EYE THE RUST RING WAS REMOVED WITH A SURGICAL INSTRUMENT
WHILE OPERATING A BATTERY MOTOR PULLING THE MANTRAIN TO THE STATION EMPLOYEE WAS THROWN FORWARD INSIDE THE CAB WHEN THE MANTRIP SUDDENLY STOPPED AFTER THE REAR MOTOR WAS CAUGHT IN THE AIR DOOR S THIS INCIDENT RESULTED IN A FRACTURE TO A RIGHT RIB
WHILE EMPLOYEE WAS ATTEMPTING TO UNCOUPLE AN E_21 MUCKER HE SIGNALED HIS PARTNER TO GO FORWARD. THE MUCKER DID NOT UNCOUPLE AND IT ROLLED FORWARD AND IT'S WHEEL CAUGHT HIS RIGHT FOOT AGAINST THE TRACK AND GUARD RAIL. THIS INCIDENT RESULTEDIN A 6" LACERATION TO THE INSIDE OF RIGHT ANKLE,REQUIRING 9 SUTURES, CAUSING EMPLOYEE TO LOSE TIME FROM WORK.
EE UNCOUPLED LOCOMOTIVE. THE REAR LOCOMOTIVE ROLLED AHEAD AND PINCHED LEFT FOOT BETWEEN LOCOMOTIVE AND COUPLER. FAILED TO FOLLOW ACCEPTED PROCEDURES AND PRACTICES WHEN DISMOUNTING LOCOMOTIVE.
EE WAS RIDING ON THE BACK OF A MOTOR. HE LOOKED DOWN AND THE AIR DOOR FRAME STRUCK HIM ON THE HEAD.
EMPLOYEE UNCOUPLED A RAIL CAR, IT ROLLED FORWARD AND STRUCK HIS FOOT.
EMPLOYEES LEFT FOOT WAS CAUGHT BY THEFIRST MUCK CAR OF A MOVING TRAIN. HE SUFFERED A TIBUAR FRACTURE.
CLAIMANT WAS INSTRUCTED TO INSPECT FAN LINE PRESSURE AND ONLY TURN FAM ON OR OFF. CLAIMANT WAS IN FRONT OF FAN STARTER BOX WITH HIS LIGHT OFF OF HIS HARD HAT SUSPENDED AROUND HIS NECK UNLIT. NO ONE COULD SEE HIM. TRAIN HAD 120' VISIBILITY WHEN INCIDENT OCCURRED. BRAKEMAN SAW HIM A TLESS THAN 10' AND SIGNALLED TRAIN TO STOP. TOTAL STOPPING DISTANCE LESS THAN 30' INCLUDES REACTIO
EE WAS OPERATING A LHD IN THE 3722-D STOPE AREA, PANEL 14S, WHEN A ROCK FELL FROM THE BACK STRIKING EE ON THE LEFT INDEX FINGER RESULTING IN A FRACTURE.
HAULING ORE TO DUMP, MOTOR JUMPED TRACK ADN CAUSED EMPLOYEE TO JUMP AROUND I MOTOR, HURTING HIS BACK.
EMPLOYEE WAS RIDING IN THE MANTRIP WHEN IT DERA ILED WHILE CREW WAS LEAVING AT END OF SHIFT CLAIMANT HIT AGAINST THE BENCH HURTING HIS BACK
EE WAS RIDING OUT ON A MANTRAIN. AS THE MANTRAIN WAS GOING THROUGH AIR DOORS THE CABLE SNAPPED ON THE AIR DOOR CAUSING IT TO CATCH THE SECOND MANCAR, BRINGING IT TO A COMPLETE STOP. HE WAS TRE ATED FOR MULTIPLE STRAINS AND RELEASED FOR REGULAR WORK. ON 10-11-94 EE WAS TAKEN OFF THE JOB & PLACED IN A WORK-HARDENING PROGRAM & IS NOW A LOST TIME INJURY.
EMPLOYEE WAS BARRING A HUNGUP WHEN A ROCK CAME OUT OF THE DRAW POINT & STRUCK HIS FOOT THIS INCIDENT RESULTED IN A FX #4 TOE LEFT FOOT CAUSING HIM TO LOSE TIME FROM WORK
EMPLOYEE WAS STANDING IN THE GO-BETWEEN WHEN THE HAULAGE BEGAN PULLING MUCK. THE DUST WAS HEAVY AND HE STOOD THERE WAITING FOR IT TO CLEAR UP. A ROCK ROLLED ONTO THE GO-BETWEEN STRIKING HIS FO OT. THIS INCIDENT RESULTED IN A FRACTURE OF HIS 4TH AND 5TH TOES, LEFT FOOT, CAUSING HIM TO LOSE TIME FROM WORK.
EE ATTEMPTED TO CLEAN CONVEYOR TABLES WHILE THE CONVEYOR WAS IN MOTION. THE FINGER OF HER GLOVE GOT CAUGHT BETWEEN THE IDLER AND CONVEYOR AND PULLED HER LEFT HAND INTO A PINCH POINT FRACTURING HER WRIST AND AVULSING A PORTION OF HER LEFT FOREARM.
TEAM MEMBER JUMPED OFF OF A MOVING TRUCK, CAUSING HIM TO FALL LANDING ON HIS LEFT HAND BREAKING HIS LEFT ARM.
A FULLY LOADED WATER TRUCK ROLLED BACKWARDS DOWN THE RAMP, HIT BERM, & TURNED OVER LANDING ON THE PASSENGER SIDE. EMPLOYEE SUSTAINED INJURIES WHEN SHE DROPPED OUT OF THE PASSENGER SIDE DOOR.
EMPLOYEE WAS LYING ON HIS BACK ATTACHING RING BEAM TO THE ERECTOR. HE PLACED HIS HAND ON THE BEAM'S FLANGED AND PULLED HIMSELF UP. BEFORE HE BECAME ERECT THE OPERATOR ACTIVATED ROTATION, PULLI NG HIS FINGER THROUGH THE ROLLER. EMPLOYEES INVOLVED IN THIS TASK NORMALLY USE THE BOTTOM RUNG OF THE STEP TO PERFORM THIS MOVEMENT.
TEAM MEMBER WAS MOVING TIMBER ON LHD A ROUND CAP ROLLED & PINNED HIS HAND BETWEEN THE CAP & THE LIGHT ON THE LHD
EMPLOYEE WAS BEING SHOWN HOW TO OPERATE A BALL CHARGE LOCOMOTIVE. HE WAS ON THE WEST SIDE OF THE LOCOMOTIVE AND MOVED TO THE NORTH--FRONT END OF THE LOCOMOTIVE TO SEE HOW THE CONTROLS WORKED. AT THAT TIME THE CONTROLS WERE ENGAGED AND THE LOCOMOTIVE WENT FORWARD SMASHING HIS FOOT BETWEEN THE STOP AND THE FRONT LOWER CORNER OF THE LOCOMOTIVE. THIS INCIDENT RESULTED IN A FRACTURE OF
EMPLOYEE WERE CHANGING A C-6 CONVEYOR MOTOR WHEN THE BEAM CRAWL SPREAD APART ALLOWING THE MOTOR, CHAIN HOIST AND BEAM CRAWL TO FALL. THE CHAIN HOIST AND CRAWL STRUCK EMPLOYEE. THIS INCIDENT RE SUTLED IN MULTIPLE CONTUSIONS AND LACERATIONS, HEAD, RIGHT SHOULDER AND BOTH ELBOWS. THE LACERATION TO HIS HEAD REQUIRED THREE SUTURES.
EMPLOYEE WAS RIDING ON THE BACK OF A BATTERY MOTOR. THE BATTERY MOTOR HE WAS RIDING IN STRUCK ANOTHER BATTERY MOTOR KNOCKING BATTERY MOTOR OFF THE TRACK CAUSING HIM TO STRIKE HIS KNEE AGAINST THE BACK OF MOTOR PANEL. THIS INCIDENT RESULTED IN A CONTUSION, LEFT KNEE. A NEEDLE WAS USED TO DRAIN THE KNEE, MAKING THIS A REPORTABLE INJURY.
TEAM MEMBER WAS RIDING AN A.T.V. "QUAD" GOING TO WHAT THE COMPNAY CALLS, INDIAN POND, TO GET A PH SAMPLE WHEN HE HIT A 10" CORLON PLASTIC PIPE, CAUSING HIM TO WRECK.

EMPLOYEE WAS DRIVING A FORKLIFT DOWN A HILL. ROAD WAS WET DUE TO RAIN AS EMPLOYEE APPLIED BRAKES WHEEL BEGAN TO SLIDE & TRANNY POPPED INTO NEUTRAL. AS FORKLIFT REACHED BOTTOM EMPLOYEE HIT A RU T OR ROCK CAUSING FORKLIFT TO TIP SIDE HE SWERVED TO AVOID HITTING CARS WHEN IT ROLLED OVER ON ITS SIDE.

TROLLELY MOTOR PULLING 6 LOADED ASEA CARS COMING DOWN NH2 ENTERED 3CX0 AND STRUCK THE TOOL CAR CAUSISNG IT TO JACK-KNIFE WITH THE BATTERY MOTAR. THIS INCIDENT CAUSED EMPLOYEE'S LEG TO BE CAUGHT BEWTEEN THE TOOL CAR AND THE BATERY MOTOR RESULTING IN COMPOUND FRACTURES, L FEMUR & TIBI

I WS WORKING AT MAGMA'S MINE, INSTALLING A PARKING BRAKE ON A 16 G MOTOR GRADER. I HAD MY KNEE WEDGED BETWEEN THE TANDEM & THE FRAME. ANOTHER MECHANIC WAS TRYING TO RAISE THE BELLY PAN. THE PA N WAS IN A BIND & THE CHAIN SLIPPED OFF & SMASHED MY LEFT KNEE AGAINST THE FAME. WE WERE USING THE BOOM ON MY TRUCK TO RAISE THE PAN.
EMPLOYEE WAS STANDING BETWEEN TWO POSTS FACING THE RIB NEXT TO THE PONYSET MANWAY. THE TRAIN WAS MOVING FORWARD AND HIS LAMP CORD GOT CAUGHT ON A CAR AND ROLLED HIM AROUND THE POST INTO THE MA NWAY. THIS INCIDENT RESULTED IN A NON DISPLACED FRACTURE, LEFT 3RD RIB.
EMPLOYEE WAS IN THE CAB OF A SUPPLY TRUCK WHEN IT WAS STRUCK BY A DERAILED ORE CAR FROM THE ORE HAUL TRAIN. THE TRUCK WAS CRUSHED BETWEEN THE ORE CAR AND THE DIRT BANK. THIS INCIDENT RESULTED IN A CRUSHING INJURY, LEFT FOOT, CAUSING HIM TO LOSE TIME WORK.

EMPLOYEE WAS RIDING OUT ON THE BACK OF A MUCK MOTOR ON 3800, HIS GLASSES MOMENTARILY FOGGED UP. HE DID NOT SEE A PIPE. HIS HEAD STRUCK THE PIPE, CAUSING 2 SMALL LACERATION.
WHILE EMPLOYEE WAS BACKING UP AN ELECTRIC HAND OPERATED FORKLIFT HE RAN OVER HIS FOOT. THIS INCIDENT RESULTED IN A SOFT TISSUE INJURY, RIGHT FOOT, CAUSING HIM TO LOSE TIME FROM WORK.
