Showing all 3While standing on a 4 x 12 board between two metal conveyor supports employee was guiding forklift operator to raise the gravity take-up on the belt to install a new belt. A fork got caught on a 3 piece of angle iron and the pressure on the fork became too great causing the angle iron to bend, and the fork to strike employee in the neck, face, and side of head.
THE OPERATOR WAS WATERING HAUL ROAD, STOPPED AT TOP OF HILL & STARTED TO BACK UP; IT WAS TOO WET AND HE LOST AIR PRESSURE, GOT OUT OF GEAR & DROVE TRUCK INTO BANK. WATER TRUCK STOPPED, THEN W ATER IN TANK SHIFTED FORWARD, CAUSING THE TRUCK TO ROLL ON SIDE.
