Worker was cleaning plant when they pinched their right 4th and 5th fingers in between bobcat bucket and hydraulic ram causing laceration of the 4th finger and fracture of 5th finger.

Employee was being trained on a Rock Drill when one of the steel drill rods on the rotating carousel became loose from the top part of the carousel star. While employee was applying pressure to the drill rod trying to push it back in place, EE smashed finger.

Employee was walking at the plant close to the 3"" minus rock pile when he stepped on a rock and twisted his left knee.

While standing on a 4 x 12 board between two metal conveyor supports employee was guiding forklift operator to raise the gravity take-up on the belt to install a new belt. A fork got caught on a 3 piece of angle iron and the pressure on the fork became too great causing the angle iron to bend, and the fork to strike employee in the neck, face, and side of head.
