Employee was cleaning up floor pit with a loader. He hit a rock which stopped the loader - seat belt came lose and the employee hit his head on the windshield and his stomach hit the steering wheel causing some bruising.
Material in a cone discharge conveyor tailpully caused the belt to stop, the plant operator radioed ee to check out the belt. He climbed up on the plant grabed the drive belts to turn the conveyor backword by hand to clean the tailpulley out. As he did the plant operator started the conveyor.His thumb went arond the Vee Belt Pulley severing the tip of his thumb.
Employee was driving a loader and hit a pothole and claims his back hurts. He went to the doctor but has continued working full time.

CHAIN SADDLE ON BUCKET OF LOADER PULLED ON CHAIN TO ALIGN SADDLE W/PICK POINT BUCKET APPROX 10' OFF GROUND WHEN CHAIN SADDLE PEEPLED OFF BUCKET EDGE AND FELL TOWARDS HEAD AREA AND IN THE PROCE SS OF WARDING OFF THE BLOW W/RIGHT ARM ELBOW WAS IMPACTED RECEIVING THE DAMAGE.
