Showing all 9Operator was opening ports on screw conveyor (6CS34) to drain water. Operator saw some buildup of material in the east port and tried to knock it out with a 10" crescent wrench. The screw caught the head of the wrench, jerking it out of operator's hand, cutting finger and thumb. Sutures were needed in index finger and thumb.
Employee was assisting another mechanic who was moving heavy metal panels with a forklift. He was positioned between panels and wall of warehouse. When forklift driver picked up pallet, the load shifted and panel fell on ee's left leg, then on to foot.
Operator was preparing to step onto manlift to descend. Operator heard a noise, turned to look & inadvertently placed his right hand on manlift track as step was moving by. Step rollers came around & smashed his right thumb, requiring stitches.
Employee was working on another piece of equipment and got his shirt tail caught in an unguarded drive shaft. Contusions to upper body and face.
OPERATING SHUTTLE CAR. EE INFORMED HIS SUPR. THAT HE HAD BEEN BOUNCED AROUND DUE TO HITTING SOME ROUGH SPOTS IN THE HAUL ROAD. EE DID NOT REPORT THIS UNTIL A WEEK AFTER THE EVENT. ALSO, INITIAL DR. EVAL. INDICATED THAT EE'S PAIN WAS NOT WORK RELATED. AFTER FURTHER REVIEW ON 11/29/05 BY THE PHYSICIAN, IT WAS DET. TO BE WORK RELATED & WAS RELATED TO THE 11/1/05 INCIDENT.
WHILE OPERATING SHUTTLE CAR 39, EE HIT RESIN ROOF BOLT WITH HEADACHE RACK, BENDING THE HEADACHE RACK & PINCHING HIS RT. HAND(RING FINGER) BETWEEN THE HEADACE RACK UPRIGHT STAND & THE OPERATOR'S COMPARTMENT SIDE BOARD.
ON APRIL 4 2001 EE WAS OPERATING SHUTTLE CAR WHEN SHE HIT A BUMP AND STRUCK HER HEAD ON THE HEADACHE RACK CAUSING HER SOME PAIN IN HER NECK. EE WAS EXAMINED BY A PHYSICIAN AND WAS RELEASED TO RETURN TO WORK.
EMPLOYEE WAS OPERATING A SHUTTLE CAR ON 6/1/00, WHEN HE HIT A BUMP AND A FILLING IN HIS TOOTH FELL OUT. EMPLOYEE DID NOT GO TO THE DOCTOR UNTIL 8/21/00.
