Employee was attempting to reset the manlift by pulling rope. When employee pulled down he scraped his hand over sharp metal and resulting in two finger lacerations. Employee received medical treatment and returned to work full duty.

Employee had bruising and redness to left shoulder and returned to work. On 11/8/12 employee received medical treatment for shoulder pain and received work restrictions.
Employee was pulling feeder breaker power cable when he stepped on a rock, causing him to fall. Received medical attention for injury to left knee.
Employee was working in the East Mine Fan Tunnel. He slipped and fell while carrying a large pry bar. The pry bar crushed the tip of left ring finger, requiring sutures to repair.

Employee was turning condensate water valve, reaching with arm above water filter canister. As employee turned valve on, canister cover blew off, striking employee's arm. Employee was treated at hospital for burn and buildup of air/fluids in upper arm. EE was able to return to work full duty.

Miners were being lowered into the mine in the cage of the #1 Service Hoist. The cage came to an abrupt stop and employee was thrown to the cage floor. The employee had soreness in knee. EE received medical care, and was given a full release but missed the next scheduled shift.
Employee was mopping the floor when she felt a pop in her thumb, she continued to mop. Then she felt a second pop in her thumb and reported it to her supervisor. Was advised to seek medical attention
Operator was adding 50 lb. bags of additives to tank. After picking last bag off of pallet, operator felt a sharp pain in the center of mid-back. Operator had soreness in back and was seen by doctor.
Employee was walking through plant to bid a project with OCI rep & 1 other Premier employee. Went into a section with a lot of steam & it was hard to see. Noticed water on floor about 10 feet into area, then felt feet burning and ran out of area. Tops of both feet were burned by hot water.
Employee was dumping waste cans into back of dump truck. One can was very heavy, so employee stepped up on hitch of dump truck to help lift can and heard knee pop. Employee received medical care.
Bolter was stuck. Employee started to climb up out of bolter compartment. Miner operator saw employee moving and lowered the continuous miner tail. Employee's hand was between bolter compartment and tail and was caught and lacerated. Employee received sutures.
While walking, at a rapid pace, around a corner employee slipped and hit the ground landing on his shoulder. His boots had some mud on them.
While entering a bobcat skidsteer employee slipped on the step pad and hit his shin on one of the forks. He received an abrasion on his shin and later complained of knee pain on his right leg.
Injured lower back while lifting and turning.
Employee was replacing side arm on a screen in the Crusher Building, and finger was pinched with equipment as it was being positioned. Employee received sutures.
Employee was adjusting make-up water to cooling tower when a hose came out of the top of the tank and sprayed employee with hot process water. Employee received burns on right side of his body.
While mechanic was cutting on roof bolter of continuous miner, sparks ignited methane that was escaping from floor. Flame was put out immediately with water hose from the continuous miner.
Employee was using a cutting torch to repair railcar when a hot piece of metal popped and a small piece went in behind face shield and safety glasses and hit left eye. Employee went to the OCI Safety Department and had his eye flushed out with sterile water. Employee went to Eye Doctor after work and the doctor gave him some eye drops.
Employee was emptying load from shuttle car to crusher feeder, when a rock flew up against canopy post and pinched employee's finger. Employee received medical treatment for injury.
Employee was greasing in #1 Crusher, got foot caught in air hose and fell backward hitting his back on a rock. Employee received medical care and was not able to return to work on 12/14/09.
Hoist #3 was taken down to perform preventive maintenance. While trying to bring hoist back online, some programming issues were found and repaired. No underground employees were trapped; two means of escape were available at all times.
Employees was dismantling scaffolding, a piece came loose and smashed his right 3rd finger, causing fractured bone and laceration
The EE was working in the coal storage area that day and had no complaints. EE went to work next day and was in the control room when he felt something in his right eye. EE and supervisior washed out his right eye, but could not get the coal out of it.
Hoist #2 went down at 2:35 a.m. Because there was only one means of escape (Hoist #1), the mine was evacuated. MSHA number (for immediately reportable accidents) was called. Surface electricians found a failed coil on the brake system and had the repair completed by 4:30 a.m.
#3 Shaft south skip hung up and is damaged. It will take several days to repair. There are two hoists available to take employees out of the Mine. MSHA was called.
Hoist #3 (south skip) had an open door at the apron feeder. Skip would not clear at the clam shell, which tripped the hoist. MSHA was called. Two hoists were still operable. Hoist #3 was back in service at 2 p.m.
Mechanic was cutting out bent bit from miner drum and a spark ignited methane gas which was being released from floor. Mechanic extinguished fire with water hose from continuous miner in less than 1 minute. MSHA hotline was called.
Main hoist motor for Service Hoist #1 malfunctioned. MSHA (800 number and Green River office) was notified at 11:45 a.m. There were 50 miners underground. Two hoists were still operable (#2 & #3 Hoists). Motor was replaced and functional at 5:10 p.m. on 8/18/09.
Employee was helping direct a motor which was being lifted by a 2/1 directional pulley rigging. Employee had left hand on stationery cable & right hand reaching for motor, when left pinky finger went into traveling pulley, removing tip of finger just below nail.
EE was going up a silo in a man basket. As he reached the work level he prepared to put on his blast hood. He noticed his rope grab was sitting on the basket & his life line had not gone through. As he started to feed the life line through it whipped back & hit him in the right eye. The strike ruptured blood vessels and tore his retina. He was not wearing his safety glasses.
Injury was lifting and moving asbestos waste bags and twisted wrong while holding the bag of about 30#s. Lumbar Strain
#2 Hoist went down due to the whisker switch sticking in "on" position. Whisker switch was repaired at 9 a.m. No mine employees were trapped; two means of escape were available.
Rock prevented clam shell from opening on ore skip of Hoist #3. Attempts were made to notify MSHA, but supervisor was unable to leave message at "800" number. No mine employees were trapped, two ways of escape were still available at all times while miners were underground.
Employee was moving miner cable with a crutch (against the rib) and felt a sharp pain in his left shoulder. Employee was able to continue working with no restrictions. Employee received surgery on shoulder in May and was unable to work on scheduled shift beginning May 29, 2009.
Employee was replacing the gear reducer on the drive of a screw conveyor and moving the suspended load. The load hit bumped toe board and pinched right pinky finger. He was able to return to work.
Employee was walking to change house at end of shift. Employee slipped on concrete walkway and fell on seat. Employee received medical care and was unable to return to work full duty.
Employee stuck welding rod in his hand while welding, received a puncture wound that got infected.
Employee was holding a sheave wheel block during a repair, while another employee was using a punch hammer and a 4 lb. hammer to free wheel. Punch hammer glanced off of wheel and struck other employee's finger tip. Employee received medical attention, finger was fractured. Employee was able to return to work with restrictions.
Employee was crouching under a pipe when his knee hit the lanyard and the lanyard swung up and hit his tooth.
While conducting a wire rope test at the drum end of #2 Shaft North Skip Rope, contractor employee put his left leg through a window cutting leg below the knee.
Employee was lifting rebar, twisted, and strained back.
Employee was removing a broken bit from continuous miner drum, using a punch hammer and a 4 pound hammer. A projected piece of metal sliced employee's forearm and was embedded. Employee was given medical attention, metal was removed and sutures given to repair laceration.
NO INJURY, employee was tramming 330DL Track hoe when he struck an overhead powerline. Powerline was severed. NO ONE INJURED. Lines being damaged caused units to trip inside of Plant.
Electrical power kicked off and electricity to both main fans on all three hoists was lost at 12:04. Power to the mine was restored to the mine at 12:23, #1 fan restored at 12:28, #2 fan at 12:45. Miners began evacuation at 12:10 but restoration of the fans and two hoists allowed miners to return to work.
Employee was installing outlet gate under railcar when outlet gate shifted on jack and pinched fingers between outlet gate and jack. Resulted in fractured little finger.
#3 Hoist tripped out with a slack rope indication at approx. 8:45 p.m. Then at 9:30 p.m. #2 Hoist stopped with a brake release problem. Mine evacuation began at 9:40 p.m. At 12:15 a.m. on 4/12/08, both hoists were repaired and ready for service.
A burner box was being hoisted away from duct work on kiln in Unit 6. As employee was pulling the chainfall, two eyebolts (connecting the rigging) failed, allowing the burner box to fall to the deck and push employee backward. Employee injured right knee and received laceration to left hand.
