miner was driving 1989 Kenworth water truck up the haul road and either shifted or stalled the water truck the truck began rolling backwards rolled into muddy conditions truck began sliding and overturned. Employee climbed out of water truck and complained of some low back pain.

Showing all 3Employee was driving a 1991 325M Dresser empty up to the top of the mountain to get a load of Gypsum Rock. The road was too wet and the truck slid down the haul road in reverse rolling into a burm and rolled on its side.
Forklift rolled over right edge of right foot. Employee was filling forklift up with propane. Strained right foot. Dr. released employee immediately to light duty. No light duty available. Employee will receive a 3 day non paid suspension and written warning. No re-check needed.
