Showing all 4Employee went to adjust gap between top roll and end of conveyor chain. Co-worker started chain conveyor. Injured's left little finger hand was pulled into gap. The conveyor stopped. Other employee came to victim and tried to pull his hand out but could not. He then got a steel bar to widen gap and pulled victim's hand free.
EE WAS BACKING UP A 35 TON DUMP TRUCK TO DUMP A LOAD OF BARITE. WHEN THE DUMP WAS RAISED THE GROUND TURNED LOOSE UNDER BACK TIRES. TRUCK FLIPPED OVER HIGHWALL AND FELL APPROX 30 FT LANDED UPSIDE DOWN. 911 WAS CALLED & EE WAS TAKEN TO ER ROOM. EE REC'D BROKEN LEFT ARM & COMPLAINED OF LOWER BACK PAIN
EE MOVING MUDBALLS W/LOADER; HEARD SOMEONE FALLING DOWN CHUTE, REVERSED TO BACK OUT OF WAY. THE PERSON HIT THE LIP OF BUCKET, BOUNCED UP AND HIT THE TOPS THEN CRASHED THRU THE WINDSHIELD. GLAS S BROKE LENS OF GLASSES ALLOWING FOEIGN MATTER THE LEFT EYE.
