Driver was reaching for a bottle of water, they failed to maintain control of the truck and drove off into the ditch on the right side, causing the truck to lay over on its side. The weather was sunny and dry .

Showing all 4The worker was reinstalling a guard on the rock screw. As the loader let the guard down. The guard came in contact with the frame of the rock screw. Causing the chain to release resulting with the guard coming down on the right foot just above the steel toe of the boot. The resulting injury is a broken little toe on the right foot.
Employee was connecting a chain from the stacking conveyor to the front end loader to reposition stacking conveyor. Loader began backing, chain tightened, and employee did not release chain. Employee strained right shoulder.
